POSITION PURPOSE
Adobe Population Health is hiring a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for both pediatric and adult members. This role focuses on improving health literacy and overall quality of life through comprehensive annual visits.
The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health.
This part-time position requires a minimum commitment of 20 hours per week. Availability can include the following:
Weekdays: Monday through Friday, 8:00 a.m. - 8:00 p.m.
Weekends: Saturday and Sunday, 8:00 a.m. - 5:00 p.m.
This position is pay-per-visit and will vary from $75-$125, depending on the type of visit. Travel reimbursement is also included at the IRS-approved rate of .70/mile.
DUTIES & RESPONSIBILITIES
Travel up to 90% throughout designated regions, which may be updated based on business needs.
Conduct in-home assessments on identified patients, following established guidelines.
Visit types may include:
In-home Medicare and Medicaid Assessments.
In-home Well-Child Visits (WCV), for children ranging from 6 months to 21 years of age.
In-home Child and Adolescent Well-Care Visits (WCV).
In-home Annual adult Wellness Visits (AWV).
In-home Health Risk Assessments (IHRA).
Telephonic Adult Annual Preventative (AAP) Assessments.
Telehealth assessments, as needed.

The GUIDE Program You'll Lead
You'll implement nine CMS care delivery domains creating comprehensive support for dementia patients and their caregivers:
Comprehensive Assessment: Dementia staging, medical/psychosocial needs evaluation, caregiver burden assessment (Zarit Burden Interview)
Person-Centered Care Planning: Individualized roadmaps addressing all identified needs
Care Coordination: Interdisciplinary team communication across all providers
Care Navigation: Supervise team members who connect patients to medical care AND community resources (meals, transportation, respite, home health)
24/7 Helpline: Human support (not AI) for patients and caregivers during crises
Caregiver Education & Support: Training programs, support groups, one-on-one guidance
Respite Services: Coordinate Medicare-funded respite care (up to $2,500 annually per patient) giving caregivers critical breaks
Health-Related Social Needs: Screen and address food insecurity, housing instability, transportation barriers, utility difficulties
Community Partnerships: Maintain CMS-approved contracts with respite providers, meal services, transportation organizations, Area Agencies on Aging

Under the general supervision of a Physician and direction of the Executive Director, the Nurse Practitioner provides a wide range of primary health care services within the Student Health Center (SHC) including assessment, diagnostic, and preventive and health maintenance services at the Palm Desert Campus. The incumbent works under the supervision of a physician using established standardized procedures and/or process protocols. The Nurse Practitioner collaborates with the SHC Executive Director and their supervising Physician(s) to develop standardized procedures and/or process protocols and scope of practice for their position. The incumbent provides a wide range of primary health care services, such as conducting physical examinations; diagnosing, treating, and managing illnesses and injuries; ordering tests and interpreting the results; and counseling patients on preventive care. This position is primarily located at the Palm Desert campus and may work occasionally on the San Bernardino campus.
Duties and Responsibilities:
Conducts comprehensive physical exams and obtains detailed medical histories
Diagnoses and treats common acute and chronic conditions, including reproductive health issues
Provides medication abortion services and counseling on STIs, contraception, and pregnancy
Orders, interprets, and evaluates diagnostic tests and lab work
Offers psychosocial assessments and referrals as needed
Administers first aid, urgent care, and emergency support
Prescribes medications and educates patients on treatment plans and preventive care
Performs minor procedures (e.g., suturing, biopsies, wound care, immunizations)
Maintains accurate medical records and ensures continuity of care
Collaborates with physicians and healthcare professionals on complex cases
Adheres to university protocols for quality assurance and infection control
Stays current on best practices relevant to student health care
May be assigned to provide lead work direction to specified SHC support staff which involves organizing and planning work, training and orienting employees, scheduling support staff, assigning and reviewing work, and providing input to employee selection and performance evaluations.
Participates in departmental meetings; engages in professional development activities, completes all department and university required trainings as assigned; performs other related duties as assigned and supporting the SHC operations
Must be willing to support San Bernardino and Palm Desert Campus.
Minimum Qualifications: Required Education and Experience
Education and Experience: Academic degree or certificate from an approved college or university-based Nurse Practitioner program or equivalent training and experience as outlined by the California Board of Registered Nursing (BRN). Incumbents must be clinically competent in a medical specialty applicable to their position and possess sufficient experience to independently perform the duties of a Nurse Practitioner within that specialty.
License and Certifications: Possess and maintain a valid California license as a Registered Nurse and a BRN certificate evidencing that the individual is qualified to use the title of Nurse Practitioner, as required by the California Board of Registered Nursing. In addition, if the incumbent is delegated the authority to furnish medications, the incumbent must possess and maintain a valid BRN Furnishing Certificate. If the furnishing procedures include the delegated authority to furnish specific controlled substances, incumbents must also possess and maintain a valid DEA number and certificate.
Required Qualifications
Comprehensive knowledge of advanced nursing and primary care principles, methods and procedures; thorough knowledge of medical theories, practices and protocols relative to the scope of practice, subject areas such as growth and development, pathophysiology, psychosocial assessment, pharmacology, nutrition, disease management, and the principles of health maintenance, applicable state and federal laws and regulations pertaining to practice as a Nurse Practitioner; and general knowledge of effective supervisory principles and practices, if assigned lead work direction.
Possess the abilities and clinical competence required to work as a Nurse Practitioner including, but not limited to, the ability to work independently in assigned areas of practice established by the supervising physician; demonstrate clinical competence in eliciting patient histories, conducting examinations, diagnosing and treating acute and chronic illnesses and injuries, assessing patient needs and making appropriate referrals; use clinical techniques and diagnostic tools; interpret laboratory and other test results; keep current with developments in the medical field pertinent to the incumbent's scope of practice; establish and maintain effective working relationships with other health center staff; be sensitive to and communicate effectively with diverse patients; and, if assigned, provide lead work direction to specified health center support staff.

Compensation and Benefits:
Anticipated Hiring Range: $58.00 - $65.00 per hour
Classification Salary Range: $27.39 - $100.10 per hour
The salary offered will take into account internal equity and experience among other factors.
