Job Duties
Assesses and establishes a presumptive diagnosis and implements treatment plans for their patient population.

Prescribes, adjusts or discontinues medications or therapies as appropriate.
Evaluates and reports to collaborating physician any circumstances or approaches to medical management or test results that present a significant risk to patient's health or well-being.
Performs procedures within the scope of practice of the physician assistant license.
Communicates with collaborative physician, patient, PCP and referring staff through telephone or other means of correspondence.
Instills confidence in patient acting as their advocate by providing safe, ethical care and health education to the patient and their family in a language that they can understand.
Interprets and follow policies and guidelines to define procedures and meet goals and deadlines.
Must have the ability see patients of all ages.

Monday - Thursday 11:00 a.m. - 7:00 p.m.
Friday: 9:00 a.m. - 5:00 p.m.

NursePractitioner - NP - Hospice - PRN - Flexible Schedule
We are seeking a dedicated Hospice NursePractitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance.
Key Responsibilities:
Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes.
Document clinical findings accurately and attest F2F encounters in patient medical records.
Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.
Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
Provide clinical recommendations to the IDG based on F2F assessment findings.
Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
May provide F2F coverage for multiple provider numbers with approval.
About You

We are seeking a Per Diem Palliative NursePractitioner to work with our hospice patients. This position is Per Diem, therefore no guaranteed hours.
The NursePractitioner will work in accordance to prescribed policies and procedures of the organization including all applicable state and federal regulations and under the general supervision and in conjunction with the Hospice Medical Director, the Advance Practice Nurse is responsible for performing patient medical examinations and palliative care visits relative to the face-to-face encounters required in order to certify patient's eligibility for the Medicare Hospice Benefit after six (6) months of hospice care. Documents visits in the patient's clinical record and the face-to-face encounter on required forms. Provides medical care services and palliative care intervention for hospice patients on an

Basic Qualifications
Education/Training/Certifications: Graduation from a master's-level program designed to educate and prepare nursepractitioners/clinical nurse specialists.
Completion of a graduate level 3-credit course in Pharmacology from a school duly accredited by a national accrediting agency approved by the Board or a pharmacology course of at least 45 hours integrated into the Master's in Nursing program
National certification requires the candidate to successfully pass an examination.

**SUMMARY OF POSITION:**
Penn State Health is seeking a skilled and dedicated NP for our progressive cardiology practice in Wyomissing, PA. serving Berks County for 50 yrs. The practice includes an experienced team of 13 cardiologists and 3 CRNPs.
The NursePractitioners provide care in both the inpatient and outpatient settings for adult cardiology patients. This role will alternate weeks in the hospital and clinic, caring for consult and follow-up patients in collaboration with the cardiologists. These patients include the entire gamut of a busy and growing cardiology practice including CAD, Structural, arrhythmia, and CHF.
After the completion of orientation the role in the outpatient practice can be tailored to an area of interest for example, CHF, structural, EP or general. This structure allows for diversity in acute care and developing long term relationships with the outpatients.
Prior experience working in cardiology is preferred. The position requires inpatient weekend coverage (4-5 hrs./day) for hospital rounds with the on call cardiologist on a Q6 week rotation. This position does not require night call or holidays.
**Responsibilities:**
+ Comprehensive assessment of cardiovascular patients
+ Develop and manage patients' plan of care in collaboration with the physicians and monitor their progress as needed
+ Admitting and discharging the procedural patient to ensure guideline-directed therapy and patient education.
+ Recommend medications, therapies, and diagnostic tests.
+ Ability to practice within the limits of their scope of practice.
**MINIMUM QUALIFICATION(S):**
