What You'll Do
As part of Strive's interdisciplinary clinical care team, the Nurse Practitioner (NP) will utilize their advanced education, skills, and judgment to deliver advanced nursing care for CKD and ESKD patients. The Day To Day
Accountable for patient outcomes and KPI's for a panel of CKD and ESKD patients. Collaborates with external providers to ensure goals, treatment, and care plan alignment.
Obtains patient history, performs physical exam, orders and interprets diagnostic tests and formulates a plan for individual patient short-term and longitudinal needs.
Presents patient cases and provides clinical support for clinical rounds and interdisciplinary team meetings.
Ensures assessment and plan of care incorporate best practices for chronic kidney care including CKD clinical care (management of all stages of CKD, co-morbidities, ESKD and support of transitions to renal replacement therapy, transplant, or conservative care) as well as participation in shared decision-making and end-of-life/advanced care planning discussions.
Serves patients and performs patient visits in multiple care settings as defined by role, including patient home, clinics (where applicable), partner MD space (where applicable), and via telehealth/virtual visits.
Serves as a liaison with dialysis facilities, PCPs, and nephrologists, providing care plan updates to help streamline the transitions in care.
Responsible for maintaining current board certification and state-specific continuing education requirements.
Adheres to expectations outlined in Strive's documentation policy
Deliver primary and urgent care visits to adult and geriatric patients with history of chronic kidney disease (in designated markets)

Master of Science in Nursing as a Family Nurse Practitioner or Bachelor's Degree as a Physician Assistant (PAs in OH must have completed 500 hours of post-graduate physician-supervised experience)
1 year of experience in healthcare
Active license, American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) certification and all necessary state credentials
Board certified family nurse practitioner (FNP-C); or physician assistant (PA-C), where eligible
Valid driver's license
Ability to prioritize and handle multiple projects and responsibilities

Manage the clinic's inventory and order supplies to maintain cost effective clinical operations
Perform quality control on instrumentation and CLIA waived testing as scheduled
Utilize the company information systems as directed and in compliance with HIPAA regulations and company policies
Support mentor program by being a resource and a role model
Keep clinical knowledge current by attending ongoing education seminars related to retail and family practice
Provide coverage for clinics within the region as needed, and as assigned by the regional management
Control costs at the clinics to meet the budgeted profitability goals
Maintain a presence in the community through active membership in local nursing organizations and through participation in community outreach programs

What You Will Do
As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services. You will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility's residents.
You will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians.
Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
Prescribe all necessary medications and treatments.
Perform comprehensive physical examinations of patients.
Develop and implement patient management policies and procedures.
Facilitate referrals to other healthcare professionals and medical facilities.
Communicate with attending physicians and/or specialists regarding patient care.
Be comfortable and proficient with documentation and CPT coding.
Monthly stability visits for long term residents
Evaluations for patients exhibiting changes in condition.

What You Will Need
Licensure Advanced Practice Licensure (APN) with an applicable state license
Must have an active license and DEA #
Previous experience in nursing or other medical fields
Minimum one-year experience in a Skilled Nursing setting (preferred)
