Job Responsibilities
The Nurse Practitioner is responsible for meeting all clinical, administrative, and financial performance metrics. This position is responsible for implementing, delivering, and ensuring that the highest quality of patient-centered care is delivered.
Provides patient centered, comprehensive primary care services
Participate in the selection of suitable treatment plans for all patients
Supervise staff to ensure the care team is providing appropriate care in accordance with their duties and obligations
Assist other care providers and extended care teams
Provide continuous availability for medical and technical questions to the employees, patient care staff, and ancillary care providers including after hours coverage
Actively participate in quality improvement programs, as well as participate in multidisciplinary quality assurance programs
Collaborate with the management team as needed to ensure improvement of patient care
Cooperate with clinical outcome reviews and measurements
Comply with all policies, procedures, and protocols
Provide general medical care and treatment to patients in the health center under the direction of a physician (depending on state)
Submits health care plan and goals of individual patients for periodic review and evaluation by physician.
Provides after-hours coverage in rotation with other market clinicians.
Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures
May refer patients to physician for consultation or to specialized health resources for treatment.
May provide occupational health services (i.e., post-offer physicals, occupational surveillance exams, medical monitoring, return to work exams and Worker's Compensation medical guidance) in some health centers
May need to perform therapeutic procedures such as injections, immunizations, suturing and wound care as necessary.
If applicable, delivers care at any clinic during scheduled work hours when necessary
Performs other duties as assigned.
Required Qualifications
Minimum of 3 years of clinical experience in Internal Medicine, Family Practice, or Occupational Health
Must be Board Certified
Must be licensed to practice in appropriate state
BLS (Basic Life Support) certification required

Facilitate return-to-work and light-duty assignments for employees with work-related injuries
Maintain current case log for workplace injuries outlining treatment plan for weekly case review

Position: Certified Nurse Practitioner (Full-time or Part-time)

Reports to: Clinic Medical Director / Family Health Center Leadership
What You'll Do
Provide comprehensive primary care services to patients of all ages (infants through seniors), including wellness exams, acute care, and chronic disease management.
Conduct patient assessments, diagnose illnesses, order/inter pret diagnostic tests, and develop and implement treatment plans.
Prescribe medications and therapies, including managing ongoing medication regimens.
Provide patient education, counseling, preventive health guidance, and support for health maintenance.
Collaborate closely with other healthcare professionals - including RNs, support staff, and administrative team - to ensure coordinated, high-quality care.
Maintain accurate patient records and documentation, including use of electronic health record (EHR) systems.
Participate in ongoing professional development, training, and quality-improvement initiatives to support our mission of compassionate, patient-centered care.
Who You Are - Required Qualifications
Master's or Doctoral degree in Nursing (or equivalent) from an accredited Nurse Practitioner program.
Active Nurse Practitioner license in the state of Pennsylvania (or relevant state licensure) and national certification (e.g., family practice or adult-gerontology).
Strong clinical judgment, diagnostic ability, and comfort managing a broad spectrum of health conditions and patient populations.
Excellent communication skills, with a patient-centered and compassionate approach.
Capacity to work independently and collaboratively as part of a multidisciplinary care team.
Familiarity with electronic health records (EHR) and documentation best practices.

About the Certified Registered Nurse Practitioner Position
Responsible for coordination & quality assurance of clinical programmatic & operational activities including diagnosis & treatment of illness, & patient education under medical protocols. Clinical:
Provides medical evaluation, treatment, and consultation services to patients of the health center.
Responds to emergencies and answers medical questions.
Obtains and documents, or causes to be obtained and documented, appropriate chief complaint, past medical, family, and social history, review of systems, examination, medications and allergies, assessment, and plan for each patient.
Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals.
Reviews ancillary test results and coordinates notification to the patient.
Provides patient education as needed.
Supervises patient care by non-health center agencies such as home health care, nursing home, or hospice.
Available to patients personally or through clinical staff to answer questions and relay information regarding their care.
Completion of appropriate paperwork/computer documentation to comply with insurance and reimbursement guidelines.
Completion of all documentation and paperwork in a timely manner.
Participates in on-call schedule.
Medical Administrative:
Participates in public relations and community outreach activities, as center shall from time-to-time request.
Provides assistance with OSHA, emergency preparedness, infection control and regulatory compliance.

BCLS required.
ACLS preferred.
Attendance required at assigned CHDC site(s)
