Perform, document and communicate results of Point of Care (POC) Testing.

Key Responsibilities:
Orthopedic Care: Perform assessments, surgery optimization, develop care plans, and provide follow-up care for patients with orthopedic conditions (e.g., fractures, arthritis, sports injuries).
Bone Health Management: Educate patients on osteoporosis prevention, management, and bone health, including counseling on nutrition, exercise, and lifestyle modifications.
Employee Health: Perform physical exams for employees to ensure their health and wellness needs are met. Treat minor outpatient medical conditions.
Collaboration: Work closely with orthopedic surgeons, physical therapists, and other healthcare professionals to ensure comprehensive patient care and treatment coordination.
Patient Education: Counsel patients on injury prevention, rehabilitation exercises, and lifestyle changes that support bone and joint health.
Documentation & Compliance: Maintain accurate, timely, and confidential patient records in accordance with healthcare regulations and practice standards.

Qualifications:
Education: Master's Degree in Nursing (MSN) with Family Nurse Practitioner (FNP) certification.
Licensure: Active and unrestricted Nurse Practitioner license in [State].

The incoming provider will collaborate with a team of five physicians, five advanced practice providers (APPs), and a well-established support staff. Responsibilities include conducting patient histories, performing physical examinations, and developing treatment plans. The provider will play an active role in patient care management by coordinating diagnostic procedures, writing medical orders, and identifying the need for specialty consultations. Knowledge of CMS Regulations and NCQA HEDIS Guidelines;

Make general recommendations to members intended to improve members' knowledge of their chronic condition(s), such as information concerning recommended testing;
Address and close identified gaps in care (disease-specific or preventive);
Recognize emergent or urgent situations requiring escalation and take appropriate action as specified in Inovalon policies, and as determined by reasonable professional judgment and ethical professional practice standards; and
Perform, document and communicate results of Point of Care (POC) Testing.
Maintain compliance with Inovalon's policies, procedures and mission statement;
Adhere to all confidentiality and HIPAA requirements as outlined within Inovalon's Operating Policies and Procedures in all ways and at all times with respect to any aspect of the data handled or services rendered in the undertaking of the position;
