Neighborhood Health Seeking NursePractitioners - FNP Neighborhood Health is an innovative network of 11 community health center in Nashville and Lebanon, Tennessee. We provide comprehensive primary care, integrated behavioral health, and dental services to almost 30,000 patients. Our patients primarily include immigrants, residents of public housing, individuals experiencing homelessness, and person living with addiction. As a federally qualified health center, we provide care without regard to health insurance or a patient's ability to pay. We also strive to improve our Patient Centered Medical Home model to ensure all patients get the right high-quality care, at the right place and the right time.
The NursePractitioner is responsible for the provision of health care services to patients who receive their care at UNHS health centers pursuant to protocols established with the Chief Clinical Officer. Such care may involve ambulatory services, night call care, and skilled nursing facility care. The NursePractitioner is expected to participate in quality assurance activity, member satisfaction, appropriate utilization of health care resources, and team meetings.
The NursePractitioner supports the UNHS mission to assure access of the underserved to healthcare, to engage patients in a meaningful healthcare experience through the patient-centered health home model, to provide high quality healthcare and to participate in controlling the cost of such care. Must be able to work in a Patient Centered Medical Home model. Must facilitate partnerships between patients, physicians and health teams with focus on care coordination and integration of treatment internally and externally. Must work diligently to assure that services are accessible, continuous, comprehensive, coordinated, compassionate, and culturally effective. Must be committed to eliminating barriers to care that is centered on the needs and convenience of patients above all other factors.
The NursePractitioner supports integrated healthcare involving behavioral health providers and dental health providers in care as appropriate. The NursePractitioner will achieve meaningful use of electronic health records each year as the standards are updated and expanded by the federal government.
KNOWLEDGE/CERTIFICATION/LICENSURE/SKILL REQUIREMENTS:
Education: R.N. and MSN education is needed in primary care
Certification/Licensure: Licensed/certified in the State of Tennessee as appropriate. Maintain basic life support certifications. Maintains current continuing nursing/medical education and requirements for privileging.
Skills and Abilities:
* Must have 2 to 3 years experience as a NursePractitioner. Have interpersonal skills to manage and develop the staff within the most immediate work group, and to participate on provider team.
* Has the analytical ability to review internal processes needing improvement and make suggestions.
* Has the verbal ability to communicate thoughts and actions to individual patients and to staff. Is able to provide the same level of care to patients and the same level of cooperation to staff regardless of age, race, color, creed, religion, sex, sexual preference, gender identification, national origin, health status, income level, or payment plan.

Qualifications
The NP reports to Regional Director.
Supports the leadership and collaborates with WSG's or facility's Medical Director. (WSG assigns the NP a supervising physician).
Practices adult acute/chronic care, med-surg, and primary care within scope of license, training, credentialing, current standards of care, including the quality of care criteria adopted by the LTC facility.
Knowledge of psychiatric care and working with psychiatric services.
Examines patient health by obtaining, updating, and studying medical histories, and implementing thorough physical examinations.
Determines abnormal conditions by administering or ordering diagnostic tests such as x-rays, electrocardiograms, and laboratory studies.
Interprets test results and develops related treatment plans based on results and evidence-based practice guidelines.
Arranges referrals, consultations, and therapeutic services, and confers with other physicians, specialists, and health care providers (Dietician, OT, PT, ST, LSW, Hospice) regarding diagnosis and treatment plans, including situations where other physicians may need information emergently.
Instructs, counsels, and engages in shared decision-making with patients/surrogates regarding: therapeutic regimens, wellness promotion, and disease prevention.
Maintains a patient safety environment by complying with the facility's procedures, rules and regulations.
Properly creates required documentation for billing submission, maintains necessary charts, documents, and records.
Acknowledges through written/verbal communication with all Inbox messages, supervisor texts, staff facility messages, and WSG phone messages/mail within 48 hours.

Required: Master of Science in Nursing (NP) with minimal 1-year experience, that is confident in the medical provider position and with delegating orders to support staff

We pay up to $150 per hour and offer flexible scheduling; you will receive compensation for each evaluation. We typically remit payment within 45 days of encounter documentation in our user-friendly EMR.
Typical Appointments
In-person appointments consist of general medical evaluations. Evaluations are performed once per veteran and typically take 30 to 90 minutes. These evaluations make a difference in the lives of veterans and directly impact their access to the ongoing care they need. There are no ongoing treatment needs for these veterans; each appointment is a one-time encounter.
We Schedule Around Your Availability.
Make your own schedule! Evaluate clients, earn a comfortable income, and be home in time for dinner. We efficiently schedule each series of evaluation appointments, which last no more than a total of eight hours each day. Our current need is for 3 days per week.
No Administrative Tasks Required

Typical Appointments

Job Description
The Medical nursepractitioner (NP) is responsible for providing medical best practices and compassionate quality care to the long-term care (LTC) population. Qualifications
The NP reports to Regional Director.
Supports the leadership and collaborates with WSG's or facility's Medical Director. (WSG assigns the NP a supervising physician).
Practices adult acute/chronic care, med-surg, and primary care within scope of license, training, credentialing, current standards of care, including the quality of care criteria adopted by the LTC facility.
Knowledge of psychiatric care and working with psychiatric services.
Examines patient health by obtaining, updating, and studying medical histories, and implementing thorough physical examinations.
Determines abnormal conditions by administering or ordering diagnostic tests such as x-rays, electrocardiograms, and laboratory studies.
Interprets test results and develops related treatment plans based on results and evidence-based practice guidelines.
Arranges referrals, consultations, and therapeutic services, and confers with other physicians, specialists, and health care providers (Dietician, OT, PT, ST, LSW, Hospice) regarding diagnosis and treatment plans, including situations where other physicians may need information emergently.
