Pay Range
The typical pay range for this role is:
$41.85 - $90.13
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

ROLE OVERVIEW
We are seeking experienced Nurse Practitioners (NPs) with telemedicine experience, strong communication skills and a passion for preventative medicine to join our per diem telemedicine team.
NPs in this role will conduct virtual Baseline Visits - 15 or 30-minute appointments designed to welcome and educate members on the Curative Health Plan. These visits focus on wellness, risk assessment, and prevention. Clinicians take a targeted medical history, review labs, and provide a care plan that emphasizes screenings, and lifestyle education.
This is a remote, per diem position with flexible scheduling, however, during January through April, 2026, a minimum of 20 hours a week for Baseline visits is required. Candidates must hold an active, unencumbered Texas NP license (additional state licenses may be requested based on health plan needs).

Perform assessments, order/ interpret diagnostic tests, and develop care plans
Prescribe medications, manage treatments, and coordinate hospice services
Conduct and document face-to-face visits for eligibility and recertification
Collaborate with physicians and IDT team to align care with patient goals
Participate in care planning and coordinate with team and external providers
Maintain accurate clinical documentation and ensure compliance regulation
Educate patients & families on medical, emotional, and spiritual needs
Contribute to quality improvement efforts and professional development
Serve as a clinical resource and liaison to community partners

Qualifications
Graduate of an accredited School of Nursing and Nurse Practitioner program
Master's degree with a minimum of one year NP experience
One year of NP experience or one year of RN hospice experience
Minimum of one (1) year hospice or palliative care experience preferred
Current nursing license in the state of practice
Current DEA license (if applicable to state regulations)
Current CPR certification
Certification from an approved advanced practice nursing certifying body, as required by the state
Nurse Practitioner Certification in Gerontology, Adult, or Family (preferred)
Excellent observation, verbal, and written communication skills
Strong problem-solving and mathematical skills
Proficient in nursing skills as outlined in the competency checklist
Demonstrated ability to counsel and communicate effectively with patients and families regarding end-of-life issues
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
