Responsibilities:
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
Administer and order point of care and lab-based testing as needed
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities

Flexibility - Visits are scheduled based on YOUR availability. We conduct assessments Sunday - Saturday from 8 am - 7 pm. You can work during the day, evening, and/or weekend based on your schedule.
Competitive Pay - Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20 - 100 visits earn $1,800 - $11,500 per month. This is based on the per assessment completion rate range of $90 - $115 depending on your state of residence.

Qualifications:
Skills & Experience That Shine
Master's Degree or commensurate experience and satisfactory completion of NP licensure
Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
Current BLS, ACLS or CPR certification

JOB SUMMARY
Delivering primary care and continuous healthcare services to PACE participants, utilizing the knowledge and skills required to assess, plan, and provide care in alignment with PACE Program standards. Works closely with the clinical care team to conduct assessments, reassessments, develop care plans, and deliver comprehensive primary care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide high-quality primary care with a strong focus on individualized evaluation, starting with a comprehensive history and physical exam, rather than defaulting to broad diagnostic testing panels.
Collaborate closely with the IDT before referring participants to specialists, ensuring referrals are necessary and that continuity of care is maintained.
Collaborate with the Medical Director to manage specialist referrals, initiate appropriate treatment prior to referral when indicated, review specialist notes promptly, and develop and communicate participant care plans that incorporate clinical judgment and consult findings to ensure comprehensive care.
Manage and monitor all of your participants' medications, aiming to exceed expectations in medication reconciliation, and medication interactions. This can be done in conjunction with the Medical Director and our collaborating Pharmacist vendor.
Stay actively informed of participant status during hospitalizations and facility stays, providing appropriate guidance to ensure safe transitions and care continuity.
Assume shared responsibility for all participants in the center, regardless of individual assignment, supporting the IDT model of team-based care.
Participate in and contribute to daily IDT meetings, offering clinical insights and collaborating on participant-centered care plans.
Ensure accurate, timely completion of all participant assessments and documentation in compliance with policy, regulatory, and quality standards.
Review and respond to labs, imaging, and consults in a timely manner, incorporating findings into ongoing care plans and communicating changes with the IDT.
Educate participants and caregivers on self-care strategies, medication use, and wellness practices, empowering them to participate in their own care.
Use the Emergency Department judiciously, prioritizing PACE resources unless symptoms are potentially life-threatening.
Accurately document diagnoses with attention to proper coding practices, supporting risk adjustment and quality reporting metrics.
Provide coverage for the 24/7 on-call rotation, addressing participant needs promptly and ensuring proper hand-offs and communication.
Contribute to discharge planning, supporting smooth transitions across care settings.
Assist with staff training and mentoring, contributing to clinical policy development and best practice implementation.
Participate in ongoing performance and quality improvement initiatives.
Demonstrate flexibility by stepping into other roles, as allowed by licensure and experience, when needed to support the team.
Maintain and report clinical resource needs to the Medical Director, supporting effective operations.

Conditions and Requirements:
A physician medical staff member must maintain a level of staff privileges at a Kentucky hospital approved by the CEO. Failure to maintain standing will result in reconsideration of employment status at RCHA. The facility and category of privileges will be determined by the CEO or his designee.
All Physicians and non-physician providers are required to attend clinic and RHCA general staff and/or Committee meetings. An attendance rate of 75% is expected, unless otherwise approved in writing by the Medical Director or CEO.
The physician or non-physician provider must participate in all Quality efforts undertaken by the organization. Further, the medical providers are expected to conform to the standards of care approved by the RHCA Board of Directors under the Quality Improvement Program, and other such programs as may be identified as appropriate. These efforts include but are not limited to requirements from the Bureau for Primary Health Care, Health Care Excel (Medicare), Medicaid and HEDIS.
The provider is expected to meet/exceed the goals and targets established for the RCHA Quality Improvement Program.
All providers must bill for all services and supplies in a compliant manner. Errors will be tracked and brought to the attention of the providers. Errors must be corrected by the providers. Physician billing errors will be discussed with the individual physician and a plan for targeted for reduction of errors will be developed. Training on coding, billing, and compliance will be provided by RHCA at no expense to the provider on at least an annual basis. Failure to reduce billing errors after training and implementation of a plan for correction of errors may be cause to require the individual to obtain additional training at the their own expense.
Failure to correct, reduce and/or eliminate errors after all steps in a plan of correction have been implemented, along with additional education/training at physician's expense, will be an immediate cause to assess base compensation level and/or employment status. The provider must cooperate with RHCA administration in achieving financial and patient care goals.
Being a “Team” player, focusing on mission, working in and assisting in the development of a collaborative approach to the care of patients and providing quality medical care are benchmarks for success.
The provider is expected to assist and be involved in projects and programs intended to improve services and the health status of residents in the communities served by RHCA. Examples include school health fairs, community screening projects, disaster preparedness and response, health professions education programs and bio-terrorism training/preparedness.
CME programs chosen by the providers must reflect the needs of the physician, RHCA and the patients. In some instances, RHCA reserves the right to direct the provider to specific CME programming such as additional coding education, improvement in QA or meeting evidence based indicators.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The ideal candidate will have experience working within a primary care health system and possess a proven ability to coordinate with community partners while affecting organizational change among providers, clinic staff and other administrative team members. This includes knowledge of governmental regulations, compliance requirements as well as the ability to implement policies, procedures, systems and objectives.

Education and/or Experience:
Doctorate in Medicine with a license to practice medicine, or a Master's Degree in Nursing with a Family Nurse Practitioner Licensure
Licensed or license eligible in the State of Kentucky as a MD or APRN
Minimum one (1) year experience providing patient care in a billable provider role preferred
Must maintain current BLS or ACLS and DEA registration (as allowed)

WORKING CONDITIONS:
Normal office environment with occasional evening or weekend work
Periodic local, state and national travel; travel between Health Center and Administrative Sites
Physical Demands: Lifting, bending, reaching, pushing, carrying, sitting, vision corrected in normal range, hearing corrected in normal range
Hazardous Conditions: Exposure to infectious diseases; potential exposure to physical violence
Machines, equipment, tools and supplies used: computer, postage machine, fax, copier, calculator, multi-line phone system
