The Nurse Practitioner is responsible for providing high-quality, patient-centered primary and/or specialty care through the assessment, diagnosis, and treatment of patients across the lifespan. Assess client and family adaptation coping skills and the need for crisis or other intervention. Accurately interpret diagnostic test. Communicate with the medical staff of any abnormalities or complications. This consultation may result in co-management or referral of client to medical management. Develop problem lists with associated differential diagnosis. Identify educational needs of the client and family and assist with teaching. Incorporate assessment finding into a plan to direct client care. Formulate a plan of care in collaboration with the client, family, physician, nursing staff and other members of the health care team which incorporates health care maintenance, teaching, and follow-up care. Appropriately prioritize client care. Plan and implement appropriate pharmacological therapies. Accurately and appropriately perform routine diagnostic and therapeutic techniques according to established protocols and standardized procedures. Initiate referrals based on client and family need. Seek appropriate consultation dependent upon client health care needs. Accurately collect, document, and communicate patient history, examination findings, clinical assessment, and plan of care in the electronic medical record and with the interdisciplinary care team.
