Job Duties
Assesses and establishes a presumptive diagnosis and implements treatment plans for their patient population.
Makes accurate and complete entries into the electronic medical record; orders and interprets diagnostic testing and consults as appropriate and prescribes appropriate medical therapy utilizing evidenced based practice.
Prescribes, adjusts or discontinues medications or therapies as appropriate.
Evaluates and reports to collaborating physician any circumstances or approaches to medical management or test results that present a significant risk to patient's health or well-being.

SERVICE/HOURS:
A. The Psychiatric - Mental Health CRNP shall provide 37.5 hours over the course of at least 3 days (subject to change) as directed with one half hour unpaid lunch unless hospital-related circumstances require otherwise. The Psychiatric CRNP may work up to 40 hours per week, but the additional hours must be arranged with the candidate on a case by case basis and must be pre-approved by the Chief Medical Officer, or designee. There is no allowance for overtime provided in the contract or this purchase order.
B. The Psychiatric - Mental Health CRNP is responsible for the care and treatment of mentally ill patients on a designated service area(s) and has professional responsibility for diagnosis, determination of treatment methods, and leadership in the implementation of treatment plans, and discharge planning. The work is performed under the general direction of a higher level psychiatrist or other administrative supervisor for clinical and administrative issues and is reviewed for conformance to current, accepted psychiatric nursing and medical principles and techniques through staff conferences, case reports, observation during performance, evaluation of results, and performance improvement activities.
C. Service to include responsibility in patient care and treatment which may include but is not limited to:
Psychiatric assessments/diagnosis.
Admission notes and evaluations.
Discharge summaries.
Directs and participates in Treatment Team functions.
Assures active treatment is occurring on assigned unit(s). Psychiatric rounds are to be completed daily on the units.
Required documentation in the chart is consistent with DHS/OMHSAS regulations, Torrance State Hospital requirements, professional nursing, and CMS standards.
Commitment/recommitment petitions and hearings, financial/medical competency evaluations and hearings, and other legal proceedings, as required.
Document psychiatric nursing care.
Performance improvement responsibilities as assigned.
Provides consultation to, consults with, and advises members of the nursing or other clinical staff and allied hospital personnel, agencies, relatives, and other interested persons on the care, treatment, and prognosis of patients.
Review of laboratory reports, test results, x-ray reports, other diagnostic test reports, physical and neurological examinations, consultation reports, and other medical/psychiatric reports in order to alert staff to any unusual circumstances.
Family intervention as dictated by clinical requirements.
Discharge planning and communication with the base service units.
Perform other related duties as required.
Responsibilities and duties will be governed and updated by Office of Mental Health and Substance Abuse Service, DHS, Torrance State Hospital, and other administrative and regulatory agencies' bulletins and directives.
The incumbent may chair or be a member of any hospital/medical staff committee or task force.
The incumbent is required to cover other unit(s) during normal working hours with no increase in hourly rate, as assigned, in cases where the attending psychiatrist is off duty and according to the number of primary psychiatric staff.
D. In all cases timeliness of responsibilities will be determined by Torrance State Hospital.
MANDATORY TRAINING:
All contract staff are required to attend orientation for a period as determined by TSH and shall be required to attend training during the normal day time business hours. Training shall be provided by TSH staff. Successful completion of training is mandatory.
Special Treatment Procedures:
Utilizes crisis prevention techniques.
Plans, directs, and provides Psychiatric - Mental Health CRNP care for the patients requiring suicidal precautions, special levels of observations, physical restraint and protective devices.
Responds to psychiatric emergencies, directs staff in providing the patients with individual instruction, assistance and support. Observes and reports changes in patient behaviors. Utilizes verbal techniques to address patient concerns and de-escalate. Participates in escorting consumer(s). Provides physical control of consumer(s) during application of physical hold.
Ensures the use of least restrictive measures and follows Hospital policies/procedures in the care of consumer(s) in restraint/special observation.
Emergency/Medical Interventions/Use of Medical Equipment:
Adheres to infection control / Universal Precautions policies and procedures.
Responds to medical emergencies, announces medical emergency, takes appropriate actions for which specifically trained. These could include cardio-pulmonary resuscitation (CPR), first aid, foreign body airway obstruction management, or other emergency treatment procedures. Following emergency, takes appropriate action according to DSH Policies and Procedures.
Responds to disaster / emergencies, e.g. fires, earthquakes, floods, hazardous materials incidents, bomb threats, utility interruptions or transportation accidents; follows basic emergency procedures as contained in TSH Policies and Procedures.

DUTIES & ESSENTIAL JOB FUNCTIONS
Work closely with Infectious Disease Attending physicians to provide evaluation and follow-up services for outpatients in the ID clinic. Treatment plans are documented as progress notes in the electronic medical records system. It also includes communication with the inpatient infectious disease team, coordination with other specialties, skilled nursing and rehabilitation facilities, and home care companies to facilitate safe transitions of care from inpatient to outpatient or long term care settings.
Provides women's health and wellness services including breast and cervical cancer screening / PAP tests and coordinating a respective Clinical Quality Improvement (CQI) project,
Provides and monitors continuity of care between patient visits and evaluation and management of emergency situations.
Provides psychosocial counseling and support to patients and families and makes community referrals as necessary.

Participates in EAC Recovery Team meetings as deemed necessary with the intent to share information related to medical services, coordination and medication management, as well as relevant treatment planning discussions.
Provide recovery based, person centered services.
Participate in developing comprehensive goal plans in conjunction with individuals and family members.
Provide service coordination, including coordinating and monitoring the activities of the individual's treatment team.
Provide input into individual and group supportive interventions and symptom management ensuring that immediate changes in the treatment plan are made, as necessary.

The CRNP is not a substitute for the Psychiatrist - however is an extender of the Psychiatrist based on high acuity of individuals served and geography covered by teams. A CRNP functions as an extender of the EAC Psychiatrist. The CRNP completes face to face contacts, ongoing evaluations & medication monitoring.
Provide input to treatment and assists in guiding treatment but consults w/the Psychiatrist prior to making any significant clinical decisions.
Consult with the Psychiatrist for any medication changes for collaborative decision making.
Performs comprehensive assessments of individuals and establish diagnoses in consultation with the EAC Psychiatrist.
Orders, performs and supervises diagnostic tests for individuals and, to the extent the interpretation of diagnostic tests is within the scope of the CRNP's specialty and consistent with the collaborative agreement, may interpret diagnostic tests.
Initiates referrals to and consultations with other licensed professional health care providers and consult with other licensed professional health care providers at their request.
Develops and implements treatment plans, including issuing orders to implement treatment plans in collaboration with the EAC Psychiatrist. However, only a CRNP with current prescriptive authority approval may develop and implement treatment plans for pharmaceutical treatments.
Completes admission and discharge summaries.

Issues oral orders to the extent permitted by the health care facilities' by-laws, rules, regulations or administrative policies and guidelines.
Prescriptive Authority - Prescribing, administering, dispensing medication.

Assess individual medication response/monitor symptoms/assess side effects.
