Fairview Health services seeks a board-certified Nurse Practitioner to join their Hospitalist team at M Health Fairview St. John's Hospital in Maplewood, MN. The main emphasis is to provide care and services to patients in the Short Stay/Observation Unit. This unit is a brand new 16-bed unit that is located in the hospital and is designed to streamline patient throughput by moving patients presenting with more complex conditions into an area better suited for their treatment. The setting is geared toward patients who require more management or attention than can be given in the emergency department, but do not need the length or level of services provided in the inpatient setting. The goal of the Observation Unit is to avoid unnecessary and costly inpatient admissions by aggressively diagnosing and treating patients' symptoms, allowing them to go home in a timely manner. The Institutional Special Needs Plan (ISNP) Nurse Practitioner is responsible for coordinated and comprehensive care of Provider Partner Health Plan (PPHP) members at designated skilled nursing facilities.
