Prescribes non-pharmacologic treatment or palliative modalities specific to patients' needs such as, but not limited to, wound care, physical therapy, oxygen therapy, respiratory therapy, dietary plans, nutritional supplements/feedings, durable medical equipment, etc. Monitors the patient's treatment by making Unit rounds daily. Collaborate with patients, family, interdisciplinary team members and the collaborating supervisor(s) in order to coordinate the patient's care and on-going disposition planning. Attends treatment team meetings to assist in the development of the ICTP. Identifies, monitors and measures clinical outcomes to determine the effectiveness of the physical health plan of care. Attends and participates at CSP meetings. Document on each patient at least monthly in a manner consistent with current documentation standards. Facilitates the patient's transition within and between health care settings, e.g. admitting, transferring, and discharging patients, and prepares all corresponding summaries and documents to support hand off communication. Complete billing forms as appropriate. Consult with supervising physician(s) at least weekly and whenever the clinical complexity indicates. Attend Joint Medical/Psychiatric Staff meetings. Respond to medical and psychiatric emergencies, during which the CRNP will announce medical emergencies, direct verbal de-escalation, prescribe medication as appropriate and follow emergency procedures until the arrival of a staff physician.
