Medication education to individuals served by the EAC and their supports.
The CRNP is not a substitute for the Psychiatrist - however is an extender of the Psychiatrist based on high acuity of individuals served and geography covered by teams. A CRNP functions as an extender of the EAC Psychiatrist. The CRNP completes face to face contacts, ongoing evaluations & medication monitoring.
Provide input to treatment and assists in guiding treatment but consults w/the Psychiatrist prior to making any significant clinical decisions.
Consult with the Psychiatrist for any medication changes for collaborative decision making.
Performs comprehensive assessments of individuals and establish diagnoses in consultation with the EAC Psychiatrist.
Orders, performs and supervises diagnostic tests for individuals and, to the extent the interpretation of diagnostic tests is within the scope of the CRNP's specialty and consistent with the collaborative agreement, may interpret diagnostic tests.
Initiates referrals to and consultations with other licensed professional health care providers and consult with other licensed professional health care providers at their request.
Develops and implements treatment plans, including issuing orders to implement treatment plans in collaboration with the EAC Psychiatrist. However, only a CRNP with current prescriptive authority approval may develop and implement treatment plans for pharmaceutical treatments.
Completes admission and discharge summaries.
Orders blood components for patients
Issues oral orders to the extent permitted by the health care facilities' by-laws, rules, regulations or administrative policies and guidelines.
Prescriptive Authority - Prescribing, administering, dispensing medication.
Provide medication monitoring and education to individuals as determined in the treatment plan.
Order medication and maintain documentation as needed.
Assess individual medication response/monitor symptoms/assess side effects.
Perform initial and ongoing nursing assessments as needed.
Assess and monitor medical issues as necessary.
Provide preventative and disease education as per treatment plan.
Provide leadership to team regarding medical symptomatology and treatment.
Assist coordination of all medical care for individuals with PCP, PHMCO, dental, vision, etc., as needed

Description of Duties:
Functions independently to perform age-appropriate history and physical for complex acute, critical and chronically ill psychiatric in-patient patients. Identifies, measures, and monitors clinical and related outcomes to determine the effectiveness and appropriateness of the physical health plan.
Responsible for a comprehensive evaluation of assigned patients including but not limited to presenting illness, current and past health history, hospitalizations and treatment, social and developmental history.
Identify factors that may affect health such as family, trauma, genetics, substance use, spiritual influences and/or environmental factors.
Knowledge of the evidence based practices and techniques used in the field of medicine and mental health, and the relationships between diagnostic findings and therapeutic methodologies.
Establish appropriate diagnostic and statistical coding in accordance with the current ICD coding guidelines.
Orders and interprets laboratory tests, procedures, imaging studies and consultations as indicated, to treat symptoms while improving functional health status under the direction of the collaborating supervisor(s).
Prescribes appropriate pharmacologic agents based on clinical indicators to treat symptoms and underlying causes of physical health disorders and assesses for any possible adverse effects.
Prescribes non-pharmacologic treatment or palliative modalities specific to patients' needs such as, but not limited to, wound care, physical therapy, oxygen therapy, respiratory therapy, dietary plans, nutritional supplements/feedings, durable medical equipment, etc.
Monitors the patient's treatment by making Unit rounds daily.
Collaborate with patients, family, interdisciplinary team members and the collaborating supervisor(s) in order to coordinate the patient's care and on-going disposition planning.
Attends treatment team meetings to assist in the development of the ICTP. Identifies, monitors and measures clinical outcomes to determine the effectiveness of the physical health plan of care.
Attends and participates at CSP meetings.
Document on each patient at least monthly in a manner consistent with current documentation standards.
Facilitates the patient's transition within and between health care settings, e.g. admitting, transferring, and discharging patients, and prepares all corresponding summaries and documents to support hand off communication.
Maintain a therapeutic relationship with patients, staff and families to promote positive clinical outcomes.
Participates in patient care as a member of the interdisciplinary team.
Complete billing forms as appropriate.
Consult with supervising physician(s) at least weekly and whenever the clinical complexity indicates.
Attend Joint Medical/Psychiatric Staff meetings.
Respond to medical and psychiatric emergencies, during which the CRNP will announce medical emergencies, direct verbal de-escalation, prescribe medication as appropriate and follow emergency procedures until the arrival of a staff physician.
Consultation with the collaborating physician and the Medical Director (or identified designee in the absence of the Medical Director) is required for a patient's consent or refusal of surgery or other specialty care recommended interventions, end of life care plan, such as but not limited to, implementation of the Advanced Health Care Directive policy, hospice care, and/or Do Not Resuscitate guidelines.
Is available to attend all medical and psychiatric emergencies during the hours of 8:30am to 4:30PM, Monday through Friday. Participates in noon (1200-1300) coverage schedule on a rotating basis if needed under the supervision of a collaborating physician.
Coverage of other units during normal work hours with no increase in hourly rate, as assigned, in cases where the attending physician is off duty and according to the number of primary care physician staff.
Performs minor procedures including but not limited to simple repair of lacerations.
Performs related work as required.
CRNP will maintain requirements for licensure.
