Provide emotional support to patients and families, addressing any concerns related to wound care and healing progress. Documentation & Reporting: Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and treatment outcomes in the electronic medical records (EMR) system. Prepare reports and summaries of patient care for medical and administrative teams, ensuring timely and thorough communication. Coordination & Scheduling: Coordinate visit schedules with the agencys administrative team to ensure timely and efficient patient care. Work collaboratively with the agencys team to adjust schedules as needed and ensure proper continuity of care. Provide updates to the agency regarding patient progress, needs, or any issues that require further attention. Compliance & Best Practices: Adhere to all regulatory, clinical, and ethical standards of care, including infection control practices, patient confidentiality (HIPAA), and safety protocols. Stay current with the latest wound care treatments, evidence-based practices, and medical technologies to ensure the highest level of care. Qualifications: Education: Masters degree in Nursing (MSN) and board certification as a Nurse Practitioner (NP). Wound care certification (e.g., CWCN, CWS) preferred, or willingness to pursue certification. Licensure: Active and unrestricted Nurse Practitioner license in the state(s) of practice. Ability to travel and provide care across various locations, including patients homes and nursing facilities. Skills & Abilities: Strong knowledge of wound care principles, infection control, and evidence-based treatment modalities. Excellent communication and interpersonal skills to interact with patients, families, and healthcare professionals. Ability to manage time effectively, prioritize tasks, and work autonomously in a mobile, patient-focused environment. Proficiency in using electronic medical records (EMR) systems for documentation and reporting. Physical Requirements: Ability to travel and visit patients in their homes or nursing facilities, which may include travel by car or other means.
