Division ambulatory clinical services are located in the UC Davis Comprehensive Cancer Center in Sacramento, C Street Clinic in Sacramento, the Placer Health Center in Rocklin and the new Cancer Clinic in Folsom. Department Specific Job Scope During the day, the physician will see patients in the clinic and evaluate and initiate therapy for those patients. The physician will be scheduled in person for six half-days/week outpatient clinic and one half-day/week (based on patient demand) virtual visits at Folsom Center for Health (or hybrid if there isn?t sufficient patient demand for a full half-day of video visits). Additionally, the physician will be scheduled one half-day/week at UCDCCC. What You Will Do: * Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including titration of insulin and advanced use of diabetes technology (CGMs/pumps). * Collaborate with Endocrinologist colleagues and Registered Dietitians specializing in diabetes treatment to co-manage a shared patient panel. * Review charts and conduct return visits for patients to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions. * Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols. * Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with Endocrinology collaborators. * Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with collaborating physicians to review cases, address clinical challenges, and provide ongoing support. * Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations. * Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine. * Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care. * Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program. Prescriptive authority as a Nurse Practitioner. Active national board certification (FNP, WHNP, AGNP, or similar). Minimum 3 years of recent experience (within the last 5 years) practicing as a Nurse Practitioner in Primary Care, Women's Health, or Gynecology. Ability to work independently and make sound clinical decisions. High proficiency and efficiency with technology (telehealth platforms, EMRs, communication tools).
