The Outreach NursePractitioner (NP) will be responsible for providing primary care to patients, helping patients navigate and access community services, and other resources. As a priority, the role will engage in activities that will promote, maintain, and improve the health of unhoused patients, while advocating and providing social support and informal counseling for homeless individuals and community health needs. As a member of the Outreach Team, the NP will engage people in health education, professional and peer support, and treatment for their medical and behavioral health conditions, via a trauma-informed and harm reduction approach. Compensation Range: This position is supported through grant funding. As a result, compensation is determined by the grant budget and may fall within a designated pay rate or range set by that funding source. The pay rate for this position is $100,000/y and is based on non-discriminatory factors such as skills and experience. This is a salaried-exempt role that is ineligible for overtime compensation. The Ideal candidate will:
Be responsible for establishing trusting relationships with patients while providing general support and encouragement
Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families
Continuously provide follow-up with patients from initial identification through closure via phone calls, home visits and visits to other settings where patients can be found
Assist patients with completing applications and registration forms

Document in electronic medical records per policy and standards
Respond appropriately and within nursing scope to emergencies that may occur in the field (i.e.: opioid overdose or other acute events)
Provide birth control and pregnancy options counseling
Train clients on the use of naloxone and dispense naloxone
Assess and counsel clients regarding opiate use disorder and medication assisted treatment including safer injection and smoking practices
Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs

Key Responsibilities
Optimal Care NursePractitioners provide primary and/or consultative professional care in a variety of care settings. The NursePractitioner will function in collaboration with physicians according to a Collaboration Arrangement. The NursePractitioner practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
In this role you will be responsible for:
Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
Determining follow-up assessments according to patient needs and best practice guidelines
Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care
Assessing health status by obtaining relevant health and medical history.
Performing a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician
Ordering, interpreting, and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and healthcare needs
Performing diagnostic or therapeutic interventions consistent with certification, law, and state medical privileges
Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency
Developing a treatment plan in collaboration with the patient and caregivers
Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment
Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
Holding case conferences with families when appropriate to advance the care plan
Meeting minimum productivity requirements based on practice standards.
Required Qualifications
Master's or doctoral degree as a Board-Certified NursePractitioner
Current State License as a NursePractitionerNursing skills as defined by accepted nursing standards
Sit, stand, bend, lift, and move intermittently
Must be able to lift at least 35lbs and bear the weight of an average adult effectively
Current and valid driver's license
Reliable transportation and valid automobile insurance coverage
Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled nursing facility settings
Demonstrates skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
Ability to handle multiple priorities, documentation requirements, and deadlines
Desired Qualifications
Post-acute or NursePractitioner experience preferred
Adult, Family, or Gerontology NursePractitioner certification preferred
Experience with geriatric patients preferred.
