You will:
Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.

Required Qualifications:
A minimum of one year experience as a Nurse Practitioner/ Physician Assistant is required. Experience in Family medicine or primary care practice preferred.
Active Family Nurse practitioner/ Physician Assistant license in good standing
Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.
Order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.
Minimum of two years of medically relevant experience or equivalent
Complete the DOT certification exam within 30 days of employment
Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish
Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).
Pass a FIT test and wear the provided respirator mask when protocol requires.
Education:

The typical pay range for this role is:
$46.03 - $99.14
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. Treatment Team participation- Provides pharmacological management services. Key Position Details:
This APP opportunity will provide coverage of cross-cover calls from the care team from both campuses providing help with all aspects of patient care, including diagnosis, treatments and consultations as part of the team.

Job Description:
Provides cross cover for admitted patients including fielding nurse calls and performing bedside evaluation as needed
Completes comprehensive patient assessments and determine diagnosis and treatment of illness.
Orders and interprets medical diagnostics in tandem with attending physician.
Conducts patient education when working to manage chronic illness or wellness care.
Consults with physician when patient needs outside of scope of practice, diagnosis or treatment plan is unclear.
Promotes communication within the team and other interdisciplinary health care staff.
Job Requirements
Master's degree completion of accredited Nurse Practitioner program and certification through ANCC
Active, non-restricted medical license in MN (or ability to obtain)
Ability to obtain and/or maintain MN DEA certification and National Provider Identifier (NPI)
Ability to meet criteria/qualifications for credentialing and hospital privileges
ACLS/BLS Tier 3 - required within 180 Days

Pay Range
Pay Range: $127,174 to $159,420 per year The pay described reflects the starting base pay range, assuming a full-time FTE. Your starting rate will depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).

Position Summary:
Twin Cities Physicians is seeking a Nurse Practitioner (AGNP or CNP) to join our collaborative care team. This role is perfect for a provider who enjoys building lasting relationships, working independently, and serving older adults in their communities. Wound care certification is highly desired but not required-we'll cover the cost of certification for qualified candidates. This is a fantastic opportunity to grow your clinical skillset while supporting a population in need of advanced care.
Key Responsibilities:
Travel to senior living communities to deliver on-site primary care
Manage chronic conditions, provide advanced treatments, and engage in end-of-life planning
Coordinate care with interdisciplinary teams, reconcile medications, and order labs
Communicate effectively with physicians, support staff, facility personnel, and patient families
Qualifications:
Licensed as a Certified Nurse Practitioner (CNP) in Minnesota
AGNP certification preferred
Experience in geriatrics and/or primary care
Valid driver's license and current auto insurance
Excellent interpersonal and communication skills
Passion for serving aging and vulnerable populations
