Attention all NursePractitioners, we are calling you to partner with us as independent contractors providing services to our Military Service members. We primarily work weekends, with some occasional weekdays under the Reserve Health Readiness Program (RHRP) in the State of on an "as needed basis".
We provide health readiness services to meet the medical and dental requirements to maintain a deployable military force for the following:
U.S. Army Reserve (USAR)
Army National Guard (ARNG)
U.S. Navy Reserve (USNR)
U.S. Marine Forces Reserve (MARFORRES)
U.S. Coast Guard Reserve (USCGR)
Air National Guard (ANG)
U.S. Air Force Reserve (USAFR)
Responsibilities:
Support Physical Health Assessments of military personnel.
Review health history, service member medical records, and medical questionnaires.
Interview military personnel to determine current and prior medical concerns.
Identify any concerns that need to be addressed immediately, inclusive of Behavioral Health.
Document interview results for readiness classification in the service member's medical record using the proprietary system.
Requirements
Supervising Physician may required depending on the state requirement
Active NP license in the state
CPR/BLS Certification
Experience with Primary Care, Urgent Care or Occupational Health - preferred
Experience with mobile medical services - preferred
Knowledge of Military systems, protocols, and terminology - preferred
Adaptable to fast paced environments, with large volumes of service members seen daily
Able to work extended hours
Able to work in various locations, including remote military installations
Able to travel, with weekend availability - preferred

Conducts face-to-face medical visits to patients in patient homes for routine visits, acute illness, health assessments and post-acute discharge for patients with complex medical conditions. (If telehealth flexibilities are extended, provider conducts video and telephonic visits for selected patients.) Must be comfortable performing in-home procedures, such as, not limited to wound care, IV/ IM medication, catheter insertion and removal.
Perform timely and accurate documentation of patients' medical information and all activity related to patient care in electronic medical record system.
Appropriately assign the correct coding for acute and chronic conditions.
Complete full and appropriate documentation to reflect the services provided in accordance with company policies and procedures.
Communicates, by phone, in person, in writing, or electronically, with PCPs and Specialists to provide comprehensive care or to make referrals, as necessary. Consults with supervising physician as appropriate.
Participate in On-Call schedule which may include after-hours, weekend and holiday calls.
Provides feedback for program development. Participates in collaborative multidisciplinary team meetings to optimize clinical integration, efficiency, and effectiveness of service delivery as required.
Maintain patient confidentiality and uphold HIPAA regulations.

Qualifications
Graduate of an accredited Master's Degree in Nursing (MSN) program
One plus (1+) years of post-graduate experience working as a NursePractitioner in an Adult, Family, Internal Medicine, Geriatric, Palliative or Hospice setting preferred
Current, unrestricted RN/NP state license and current state driver's license
DEA License and furnishing privileges with the state board
Board Certified through the American Academy of NursePractitioners (AANP) or the American Nurses Credentialing Center (ANCC)

This is a hybrid position, where you will work predominantly out in the field in patient homes and also from home. Providers can expect an average of 5 in-home visits per day.

The base pay range for this role is $70/hr - $82/hr. This salary range represents our national target range for this role. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.

Provide primary care and evaluate and treat acute illnesses in patients in collaboration with Physicians and Community-Based Physicians, both in the clinic and in the patient's homes.
Perform comprehensive assessments on patients in coordination with the interdisciplinary team to develop comprehensive care plans.
Collaborate on discharge planning from acute or long-term care settings.
Refer patients to medical specialists as indicated, in consultation with a Physician.
Manage care of patients in nursing homes, making regular visits as required by clinic standards and patient needs.
Actively participate in the interdisciplinary team, communicating changes, collaborating on care planning, and coordinating 24-hour care delivery.
Ensure effective care coordination in collaboration with Physicians, Community-Based Physicians, and other team members.
Participate in rotating night and weekend call coverage.
Review outcomes and utilization data to establish best practices and make policy and procedural improvements alongside the Medical Director and clinical staff.
Protect privacy and maintain confidentiality of all company procedures and information about employees, patients, and families.
Education and Certification:
Graduate of an accredited school of nursing.
RN State License.
Certification as a NursePractitioner with prescriptive authority.
Current Basic Life Support (BLS) training.
Current driver's license and proof of auto insurance.
Skills & Experience:
Minimum of 1 year of experience as a NP specializing in adult chronic disease management and functioning in a Primary Care Provider role, with or without MD oversight. Experience with the frail elderly population is preferred.
Knowledge of chronic care/geriatric issues and best practices. Experience with deprescribing and palliative care is a bonus.
Strong relationship-building skills with other providers and an interdisciplinary team.
Commitment to improving the health, experience, and value for patients.
Quick learner with new EMR and health tech communication platforms.
Excellent clinical, organizational, and communication skills. Experience with difficult goals of care conversations is preferred.
Working knowledge of regulations is a bonus but not required.

The Role:
