Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual patient's needs.
Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
Notifies the supervising RN with requests for re-authorization of “more visits needed” before third-party insurers' authorized number of visits expires.
Suggests Community Services that may be available to the patient to assist in safe home care needs.
Participates in the Performance Improvement Committee process as requested.
Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function.
Participates in the growth of the office by being a willing preceptor for newly hired same-discipline employees.

Document clinical findings accurately and attest F2F encounters in patient medical records.
Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.
Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
Provide clinical recommendations to the IDG based on F2F assessment findings.
Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
May provide F2F coverage for multiple provider numbers with approval.

Job Summary:
A Licensed Practical Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors and teaches and educates patients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RNs, Administrator, and Director of Professional Services.
Qualifications:
Graduate of an approved LPN nursing school
Currently, licensed in good standing in NV.
A minimum of one (1) year of experience as a licensed practical nurse, preferably in a home care setting.
Must have a criminal background check
Must have current CPR certification
Responsibilities:
Maintains the nursing frequency established by the supervising RN.
Follows the plan of care established by the supervising RN.
Reports to the RN.
Prepares equipment & materials for treatments observing aseptic techniques as required.
Performs home safety check and environmental check of the patients home environment on all visits. Reports any unsafe or potentially unsafe condition to the supervising RN.
Performs nursing visits and documents accordingly.
Reviews and follows the supervising RNs plan of care, communicating frequently with the supervising RN on any suggested changes necessary.
Collaborates with the supervising RN on a weekly (or more frequent basis) to benchmark toward the established outcome goals.
Review the patients ADL and ADL abilities.
Teaching patients & family self-care techniques.
Communicate any suggested changes to the supervising RN.
Reinforces the HHA plan of care when HHA services are ordered.
Effectively communicates with patient/family the POC and progression of such. Keeps the patient informed ongoing.
Effectively communicates with other disciplines in the case (actively participates in case conferencing) to effectively and appropriately problem-solve as situations arise.
Communicates effectively with the supervising RN, scheduled visits, and changes to the schedule every week. The caseload is self-scheduled but communication of the clinicians schedule to the nursing supervisor is essential.
Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual patients needs.
Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
Notifies the supervising RN with requests for re-authorization of more visits needed before third-party insurers' authorized number of visits expires.
Suggests Community Services that may be available to the patient to assist in safe home care needs.
Participates in the Performance Improvement Committee process as requested.
Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function.
Participates in the growth of the office by being a willing preceptor for newly hired same-discipline employees.
Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the patients serviced.
Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the events occurrence.
Submits written time requests 2 weeks or more in advance of planned time off.
Other duties as assigned.

Indoors, performing patient care assessment/tasks in the patients residence

At Geriatric Specialty Care of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they're most comfortable-creating a seamless, convenient, and personalized process for seniors and their caretakers.
The Role
The Provider is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH residing in assisted living communities, residential care homes and private residences. This care includes-
Obtain history and physical on new patients
Order/interpret diagnostic tests
Diagnose and treat chronic conditions and episodic illness
Perform Advance Care Planning
Accurate and timely documentation in the EHR
Coordinates with other members of the care team, both internal and external partners
In addition to the above, the Provider is responsible for the annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines. The assessments may be done in the home or clinic setting. This service includes:
Conducts Health Assessment, including past medical history, review of systems, medication reconciliation, and physical exam
Review and discuss the need to close care gaps and quality measures
Perform/review standardized screening exams
Identify all existing medical conditions
Accurately and timely documentation of assessment and conditions to include quality MEAT Statements
Identifies patients in need of chronic care management
Identifies patients in need of pharmacy assistance
Document Elements of Personal Prevention Plan and health issues to follow up with PCP
The Provider is also responsible for ensuring a safe and smooth transition of patients following an acute or skilled stay or ED visit. The care included is-
In-home post-acute visit including history and physical and medication reconciliation
Short-term management of episodic illness related to acute or skilled admission
Care coordination with team members for the implementation of a safe discharge plan
Identify barriers/obstacles to safe, self-management in the community and work with care team to overcome or identify alternatives
What We're Looking For
Current Nevada APRN license, credentialed with Medicare, Medicaid and health plans PPH is actively contracted with (Senior Care Plus, Prominence, OPTUM,) or has the ability to be enrolled with any health plans.
Minimum two years experience in specialty care area (Geriatrics) in private physician office, clinic setting or community-based program preferred.
Demonstrates clinical competence including strong communications skills and medical knowledge.
Remains current in licensure according to state regulation.
Positively represents the practice by communicating to all visitors, patients and staff in an enthusiastic, friendly and professional manner.
Provides a positive environment and experience with patients
Actively seeks out methods to support the practice, staff, patients and physicians

Nature and Scope
Nurse practitioners are primary health care providers who practice in ambulatory, acute, emergency room, critical care and long-term care settings. According to their practice specialty these primary care providers provide nursing and medical services to individuals, families, and groups. In addition to diagnosing and managing acute episodic and chronic illnesses, nurse practitioners emphasize health promotion and disease prevention. Teaching and counseling individuals, families, and groups are a major part of nurse practitioner's role.
The incumbent must provide optimal patient care through assessment, planning, implementation, and evaluation of neonatal, adolescent, adult, and geriatric patients and families, as applicable. Demonstrates the knowledge and skills necessary to provide care, based upon physical, motor/sensory, psychosocial, and safety appropriate to the age of the patient served. A Nurse practitioner performs the following acts in addition to the ordinary functions of a registered nurse if he/she is properly prepared and the acts are currently within the standard of medical practice for his specialty and appear in their protocols:
1. Systematically access the health status of persons and families by:
