Custom developed technology driven solutions for psychiatry including an integrated EMR which supports provider-to-patient messaging, telemedicine, direct lab integration and proprietary tools to track and gain insight into your patients' outcomes.
Spend time with your patients. Minimum of 60 minutes for intakes and 30 minutes on follow up visits. Our model is built upon providing supportive therapy, in addition to medication management, to our patients.
Strong in-house referral network that allows you to refer patients to specialists and therapists whose practice supports the patients you are actively treating. Best of all, clinical information is shared seamlessly through our EMR.
Your Qualifications:
Board Certified by the ANCC as a PMHNP
A minimum of 4 years' post grad experience practicing as a PMHNP, preferably in a telehealth setting; Prior RN experience in psychiatry/behavior health is a plus!
Ability to obtain and maintain active and unrestricted license and DEA number. We can assist in obtaining licensure as needed.
You must also have demonstrated:
Experience administering initial mental health evaluations, developing and implementing treatment plans, medication management, as well as psychoeducation/supportive therapy.
Proficiency in using an Electronic Medical Record system, ensuring accurate and up-to-date electronic health records for all patient encounters.
Passion for high quality care, and the ability to prioritize best practices in the virtual telepsychiatry space.
Excellent clinical knowledge and communication skills.
A willingness and passion to expand clinical skill set through continuous education, and a commitment to evidence-based practice.

QUALIFICATIONS

Optimal Care NursePractitioners provide primary and/or consultative professional care in a variety of care settings. Requirements:
Education:
Graduate from an academic program, within the nursing specialty field of
study that is accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
Master's degree in nursing
License:
Current, full, active, and unrestricted license as a Registered Nurse in the State of Michigan
Experience:
As required to meet clinical competency requirements specified in the
credentialing instructions
Certification:
Certified in the specialty required by the TO as a NursePractitioner by the
American Nurses Credentialing Center (ANCC), American Academy of NursePractitioners (AANP) or the recognized national nursing certification organization for the specialty area of practice as appropriate (for example, National Certification Board of Pediatric NursePractitioners and Nurses, American Nurses Association, National Certification Corporation for the Obstetric, Gynecologic and Neonatal Specialties)
BLS certification (American Heart Association, American Red Cross)
Job Duties to in included but are not limited to the following:
Provide a full range of outcome-based nursepractitioner services in accordance with privileges granted by the MTF (e.g., supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated)
Technically direct and teach other staff, provide evidenced-based educational lectures and participate in in-service training to staff members
Diagnose and treat patients with common acute conditions, chronic illnesses, or minor trauma within accepted protocols, clinical practice guidelines, Nurse Practice Acts, and/or in consultation with a physician
Promote preventive care and health maintenance including annual physicals, positive health behaviors and self-care through both formal and individual education and counseling
Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization
Recognize conditions that require isolation and ensure universal precautions are used in all patient encounters
Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement and safety.

Job Description:
The outpatient nurse coordinator for cardiac rehabilitation is responsible and accountable for patient evaluation: disposition and decision making in coordinating and rendering professional nursing care to adult patients with cardiovascular disease. This nursing staff member coordinates the individual treatment plan (ITP) for patients undergoing cardiac rehabilitation therapies.
Minimum Qualifications:
Qualifications/Knowledge, Skills & Abilities (Indicate minimum required to qualify or perform this job.)
Required Education/Experience/Licensing:
• Current nursing license in State of Ohio
• Three (3) years' clinical experience in care of patients with cardiovascular disease
• Certified in BCLS and ACLS

• Excellent communication skills in the area of patient education and counseling
• Competent in the performance of a cardiovascular and pulmonary history and physical exam
• Competent in quickly recognizing, identifying and treating abnormal EKG's and cardiac dysrhythmias
