Job Summary
The MGB mobile health services is an innovative program dedicated to increasing access to quality healthcare for historically marginalized communities in the North Shore area. The team in their mobile health van goes to different communities for pop up clinics to bring the care to where the community can best access it. The Nurse Practitioner is an integral member of the team bringing these health care services which include cardiometabolic screening and treatment, substance use disorder screening and treatment, vaccinations, and social determinants of health screening and resources
The Mobile Clinical NPTM is an integral member of the mobile health team that is dedicated to increasing access to quality healthcare for vulnerable communities in the greater Boston area.
Mobile health services are specialized by "clinic" and provide primary care to patients of all ages including COVID-19 testing and vaccination, hypertension screening/treatment, and substance use disorder screening/treatment.
The Clinical NPTM is responsible for providing oversight and supervision of the clinical members of the mobile team (RN, MA, and others). The Clinical NPTM also provides direct patient care to patients of all ages.
Together with the Mobile Operations Manager, the Clinical NPTM manages 1-2 clinic sessions each Wednesday across various locations that require set-up, staff management, and patient service. Because each clinic session takes place in community locations with no immediate clinical back-up, it is vital that the Clinical NPTM is experienced and proficient at preventing or managing complex and/or emergency patient care problems for patients of all ages.
The Clinical NPTM will work closely with the Senior Medical Director, Medical Lead and the Mobile Operations Manager to develop and expand mobile health service offerings in the community. It will be important for the Clinical NPTM to have an interest in Community Health and be willing to explore new opportunities for providing service.

Clinical Job Responsibilities include but are not limited to the following activities:
1. Functions independently and uses EPIC to provide care for all patients seen by the mobile team.
2. Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or emergency patient care problems.
3. Maintains and practices in accord with practitioner guidelines per state and hospital regulations.

* Collaborates with physicians in managing acute and long-term medical needs of patients.
* Provides clinical results and consultation to patients over the phone in follow-up to care provided during the mobile clinic.

QUALIFICATIONS
Physician Assistant: Master's degree in Physician Assistant Studies with certification by the NCCPA.
Family Nurse Practitioner: Master of Science in Nursing - Family Nurse Practitioner.
2+ Years of full time Emergency Medicine experience as of their projected start date at ZoomCare. The 2+ years must be practicing as a full time Physician Assistant or Nurse Practitioner Emergency Medicine (may consider candidates with 1 year of formal academic emergency medicine training on a case-by-case basis).
Recent experience in an emergency medicine setting within the last 5 years, preferred.
Current BLS for Healthcare Providers certification from a recognized and reputable organization (e.g., American Heart Association, Red Cross) or the ability to obtain one within 1 month of hire.
ACLS certified.
Vaccinated against COVID-19, Hepatitis B., MMR, PPD, Varicella (Chickenpox), and TD/TDAP.

Oregon Integrated Health is an Integrated Primary Care Medical Home that is hiring for a Full Time Nurse Practitioner for our Clinic in Salem Oregon. We will be moving into a larger space January 1st 2025, currently a Tier 4 Patient Centered Primary Care Medical Home, Vaccine for Children Program and candidate for Rural Health Clinic designation! We have an amazing team and looking forward to our growth in Salem!
Our Salem clinic sees a demographic of Medicaid and Medicare individuals that are in need for our expansion with community resources.
Our Integrated Medical Home in Salem currently has a team of Family Nurse Practitioners, Naturopathic Physicians, Licensed Professional Counselors, Behavioral Health Clinicians, Chiropractor, Acupuncture, Medical Assistants, Front Desk and community health worker with our Senior Director of Clinical Operations. Our patients have access to Interpath Laboratories directly in our facility.
We utilize telemedicine visits for patients that are at home as well as telehealth visits when patients may not have access to data or smart phones patients come to our Portland facility and utilize our technology to connect with any of our providers across our 4 locations! Our additional locations in Florence, Salem and Portland are comprised of Family Nurse Practitioners, Naturopathic Physicians, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage Therapy and oral health with an Expanded Practice Dental Hygienist.
JOB DETAILS
$12,000 Sign On Bonus, paid first pay period following 90 days of employment with OIH
Salary $135,000-$140,000 based on Experience and FTE
Flexible with hours and scheduling
Health, Vision, Dental Benefits paid at 50% of Base Plan offered by OIH, eligible after 60 days of employment
401K Benefit: OIH matches 100% of your contributions up to 3% of your compensation, plus OIH matches 50% on your deferrals that are greater than 3%, but less than or equal to 5% of your compensation (Eligibility Requirements: 1yr employment, 1000hrs worked, at least 21yrs of age)
Eligible for OIH Benefits after 90 Day Probation Period
State Medical License and DEA 100% reimbursed, completing 90 days of employment
Continuing Education Benefit up to $700 annually, completing 90 days of employment
8 Holidays Per Year:
7 Clinic Closures, 1 Floating Holiday, eligible immediately upon hire
PTO 160 Hours Per Year:
Vacation PTO accrues based on time worked, with a maximum annual accrual of 80 hours per FTE 1.0, available for use after 90 days of employment
Frontloaded PTO is uploaded based on time worked, with a maximum annual eligibility of 80 hours per FTE 1.0, available for use after 90 days of employment
Protected Sick Time accrues 1 hour for every 30 hours worked, max use of 40 per year, available immediately as time accrues after first day of employment
JOB DUTIES AND RESPONSIBILITIES:
The Primary Care Provider will provide primary care in an integrated setting with various other specialties at the clinic.
Primary Care Provider will have access to Medical Assistants that will room patients, collect vitals, submit referrals and perform electronic orders as submitted by provider.
Primary Care Provider will have access to Patient Health Navigators that will follow up on ED visits, referrals, complex care patients and quality metric data.
New patient visits are 45 minutes and follow up 30 minutes, Urgent appointments are 15 minutes. Same day appointments will be available for each provider.
Conduct standard medical and physical examinations and perform standard diagnostic tests in office and through telemedicine conferencing.
Orders, reviews and provides patient instructions regarding diet and activities and their modifications as appropriate.
Refer patients to other health professionals in the treatment of diseases requiring further diagnosis and evaluation.
Provides appropriate medical record documentation in an EHR on all patients in accordance with clinic policy.
QUALIFICATIONS, SKILLS AND ABILITIES
Previous experience in an Urgent or Family Care, Emergency preferred
Comfortable and skilled with electronic platforms such as EHR
Accredited Medical School Graduate.
