Returns patient phone messages at designated times throughout the day; follow through with appropriate action; documents action taken in patient chart. Physical requirements Position requires full range of body motion including handling and lifting patients, as well as manual and finger dexterity and eye-hand coordination. Employees will occasionally be asked to lift and carry items weighing up to 30 pounds. Position requires lifting associated with patient transfers (up to 50 lbs.) Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed and run errands as requested. Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer's physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. The Care Promoter 1 is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter 1 vary from setting to setting, depending on the size, location and type. In addition to administrative duties, Care Promoters perform clinical duties under direct physician supervision in accordance with state medical practice acts. The Care Promoter 1 is an integral member of a Physician-lead Care Team that is focused on providing excellent and comprehensive primary care for a specific population of patients. Job Description ESSENTIAL JOB DUTIES/RESPONSIBILITIES: * Measures and records patient vital signs; records patient interview and medical history. * Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel. * Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient. * Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel. * Perform non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic waived lab tests. * Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision. * Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times. * Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician. * Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. * Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls. * Performs other duties as assigned and modified at manager's discretion. Alden Village North - Provides residents with daily nursing care in accordance with current federal, state, and local standards, guidelines and regulations, facility policies and as may be directed by the Charge Nurse, Supervisor, Assistant Director of Nursing, Director of Nursing or Administrator to ensure that the highest degree of quality care is maintained at all times. QUALIFICATIONS Successful completion of a nursing arts course in an accredited nurse training program as evidenced by a diploma, certificate, or other written verification from the school and successful completion of the Department approved nursing assistant competency evaluation or successful completion of a United States military training program that includes the content of the Basic Nursing Assistant Training Program Be at least 16 years of age, of temperate habits and good moral character, honest, reliable, and trustworthy. Provide evidence of employment or occupation, if any, and residence for two years prior to initial employment as a nursing assistant. Must possess a high school diploma or G.E.D. Assists customers with packing personal possessions when they are transferred to a new room or when being discharged. Participates in and receives the nursing report upon reporting for duty. Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed and run errands as requested. Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer's physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer's (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident's eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. Wears transfer belt at all times and utilizes as needed. Keeps the nurses' call system within easy reach of the resident. Reports all hazardous equipment and conditions to the Staff Nurse immediately.
