The Patient Safety Observer/Transporter role is required to always maintain an unobstructed view of the patient; this includes when in the bathroom and when transporting the patient to and from all sub-section areas of the hospital. This role monitors the safety of the patient and reports any changes to the Registered Nurse overseeing the patient's care.
KEY RESPONSIBILITIES:
The Patient Safety Observer maintains constant observation of the patient, facing the patient, and does not leave the patient unattended (including when visitors are present) and does not leave without RN approval and relief.
Receives, reviews, and discusses the RN's report by detailing any special needs/monitoring.
Immediately reports any observed changes in the patient's behavior or condition to the RN via the intercom system or other available mediums. Informs the RN of any patient complaints related to their health.
Calls the appropriate code if needed.
Performs patient assistance activities as related to ambulation and mobilization safely, including, but not limited to, assisting patients out of and returning to their bed/chair primarily for bathroom needs (if monitoring only one patient).
Regularly conducts room safety sweeps per policy and ensures that potentially hazardous items are removed whenever possible.
Monitors applied restraints to ensure patient safety and comfort.
Assists in dressing and undressing the patient as needed and securing clothes and belongings as per company standard to ensure the patient's safety.
Maintains a professional and safe appearance, to include not wearing any items around the neck that could be used as a weapon.
Documents all observations on the patient safety sheet as instructed and within designated timeframes.
Always ensures patient confidentiality, and refrains from discussing patient information with anyone other than the assigned nurse and physician.
Always communicates in a professional and empathetic manner with the patient, by listening and acknowledging patient's complaints and refers such complaints to the RN. Any complaints that cannot be resolved at the point of care shall be referred to the patient advocate. Refrains from conversations with patients regarding their medical condition or plan of care.
Solely dedicated to observing the patient throughout the shift. The Patient Safety Observer/Transporter shall not bring personal items such as phones, food, books, or magazines into the room.

2. Gives, removes and cleans bedpan.
3. Prepares residents for meals, assists residents to eat.
4. Assists the Nurses in the care of residents as directed.
5. Reports all observations concerning residents to a Team Leader.
6. Makes beds, occupied and unoccupied.
7. Assists patients in moving and turning, and getting in and out of bed, and walking as needed.
8. Keeps water pitchers clean and filled with fresh drinking water.
9. Assists in keeping department neat and clean by: cleaning bedside unit, cleaning nursing equipment, keeping service areas in order (utility room, linen room, and kitchen).
10. Assists in keeping residents' flowers fresh and watered.
11. Charts pertinent information on flow sheet as directed by Team Leader.
MINIMUM QUALIFICATION REQUIREMENTS:
· Certification or eligible for certification in the State of Florida.
· Preference given to the applicant who can provide evidence of previous satisfactory experience as CNA.

b. A.M. care - faces and hands washed, teeth or dentures cleaned.
c. Bathroom or bedpan before eating.
d. Sit residents up in bed with over bed table in place or out of bed to chair.
e. Pass trays - assist as needed - fix eggs, butter toast, fix coffee etc.
f. Feed residents as needed - report any difficulty in eating or change in appetite to nurse.
g. Remove all trays from rooms to be returned to the kitchen.
3. Get linen ready.
4. Daily baths and showers to assigned residents. Provide privacy as per regulations. Knock on doors before entering rooms.
a. Mouth care - teeth or dentures daily.
b. Back care - back rubs, skin care
c. Peri-care.
d. Report to nurse any unusual findings with resident.
e. If resident needs any treatments for decubitus (wound care etc.) the nurse will do these - tell her when resident is ready.
5. Residents out of bed. Check those who need to be ready for therapy.
a. All residents are to be out of bed - unless otherwise ordered by physician.
b. All residents are to be dressed appropriately (no holes/rips or torn clothing).
c. Residents should be taken to TV room, dining room, activities, physical therapy room or outdoors as tolerated.
d. Complete all restorative care as ordered.
6. Assist with admissions and discharges as necessary.
a. Introduce self. Obtain height and weight of resident.
b. Bring resident to room and assist to undress and unpack.
c. Fill in clothing list and admission sheet completely.
d. Escort all discharge residents out of building.
7. Make beds using covered barrels for dirty linen. No linens on floor.
8. Assist with lunches (as with breakfast). Encourage residents to eat in the Dining Room.
9. Report to dining room at 11:30. Prepare residents for lunch. Take assigned residents to the dining room. Ambulate to dining as ordered. Sanitize resident's hands; offer clothing protectors, or assist resident eating in rooms as assigned. Monitor dining room as assigned.
a. Pass trays - assist as needed.
b. Feed residents as needed - report any difficulty in eating or change in appetite to nurse.
c. Assist residents eating in rooms as assigned; remove all trays from rooms for return to kitchen.
10. Take breaks and lunches as assigned. If one person goes late it holds the next person up.
11. Bring residents to activities in the afternoon.
12. Walk residents as per schedule and to and from dining if able.
13. Check residents every two hours and provide for toileting, restraint release, change of position tec.
14. All residents are to receive fresh water and nourishments per policy. Change water sets and clean ice chest per policy.
15. Review CNA instruction sheet for accuracy - notify nurse of changes as necessary.
16. Prepare residents for care conferences. Take them with you to conference as assigned.
17. Afternoon duties:
a. Special assignments-utility room, linen, closets, etc.
b. Make sure resident rooms are neat - bedside tables clean, bedpans and urinals clean and put away. Closet draws are tidy and neat, clothing on hangers.
c. Empty catheters.
d. Make all empty beds in your assignment and prepare room for new admission.
e. Complete all charting; flow sheets, restorative documentation etc. Notify nurse of any changes or problems.
f. All personal items in plastic bags labeled with resident names.
18. Make final rounds and report off to Charge Nurse notifying her of any resident care issues or needs.
