This is a 24-hour facility. Responsibilities:
Follows the services outlined in each resident's service plan in a friendly and supportive manners. This includes passing resident medications in accordance with physician orders and as stated on the Medication Administration Record for all residents.
Documents completion of tasks delivered on a daily basis.
Documents observed changes in condition in accordance with local rules and regulations and policies.
Responsible for communicating with other Medication Aides about resident needs for consistent quality.
Participates in meal service including serving, clearing, and cleaning the dining room.
Manages staff under their supervision during working hours.
Maintain the proper certifications required by local and state licensing agencies which include on-going in-service trainings, accreditations, specific coursework and certifications.

Should workplace injuries occur, when required, assists the Executive Director and Director, Health and Wellness with the investigation, return to work, and management of the injured worker communication with care provider and the Management Company.

Candidates should possess strong GI experience as well as exposure with internal medicine knowledge. Essential Job Duties - GI Focus:
Patient Assessment and Diagnosis: Nurse Practitioners and Physician Assistants (NPs/PAs) will partner with CMO to perform comprehensive GI-focused health assessments, including detailed medical histories, physical examinations, and diagnostic evaluations relevant to gastrointestinal disorders. NPs/PAs will formulate differential diagnoses and develop appropriate treatment plans in collaboration with supervising physicians.
Treatment and Management: NPs/PAs will collaborate with GI CMO to provide specialty care services, including prescribing GI-specific medications, ordering and interpreting GI-related diagnostic tests (e.g., colonoscopies, endoscopies, liver function tests), and implementing therapeutic interventions. They will manage chronic GI conditions, monitor patients' progress, and adjust treatment plans to optimize digestive health outcomes and patient quality of life.
Health Promotion and Disease Prevention: NPs/PAs will educate patients and families about GI health promotion strategies, disease prevention, dietary and lifestyle modifications, and long-term management of chronic GI conditions. They will emphasize preventive care, including colorectal cancer screenings, hepatitis vaccinations, and liver health monitoring.
Collaborative Care: NPs/PAs will work closely with interdisciplinary healthcare teams, including gastroenterologists, hepatologists, primary care providers, dietitians, and nursing staff, to coordinate comprehensive patient care. They will consult with specialists, refer patients appropriately within the GI specialty or to external providers, and ensure continuity of care across inpatient and outpatient settings.
Clinical Care: NPs/PAs will be required to perform clinical skills relevant to gastroenterology, including but not limited to phlebotomy, IV insertion, EKGs, and the management of patients undergoing GI procedures. They will independently manage patient encounters for both acute and follow-up GI care as needed.
Documentation: NPs/PAs will maintain accurate and up-to-date electronic medical records, including detailed documentation of GI-related patient histories, physical findings, assessments, diagnoses, treatment plans, procedure reports, and follow-up notes. They will ensure compliance with legal, regulatory, and organizational documentation standards.

Documentation of Medication Administration: Assure that all medication administration is documented by the individual assisting with administration and follows all applicable regulations. In the event of resident absence or resident refusal of medications, assure that this absence or refusal is documented in compliance with all applicable regulations.
Medication Supply: Assure that there is an adequate supply of medications on hand for each resident. Specifically, in the event less than 3 day supply of medication, order refill, send in new prescription, or refer problem immediately to the assigned caseworker.
Medication Changes: Changes are made only when there is a prescribing practitioner's written order, or when there is a

r in place. If there is a new medication prescribed, assure that the pharmacy gets the prescription and delivers new medication. If there is a medication with dosage change and the actual pill changes, assure pharmacy gets the prescription and delivers the new medication (making sure to pull the supply of the previous dosage). If there is a medication with a dosage change, but no new prescription (for example, the doctor changes the order from 2 pills twice a day, to one pill twice a day), the MAR is changed to reflect the change and assure that the prescription container is tagged with the change.
Incontinence Supplies: Assist residents with monitoring and management of supplies used to assist with incontinence issues. Work closely with outside providers, insurance companies, pharmacy, Fleming House Social Service and PCA staff to ensure that residents are receiving proper amounts of supplies at appropriate times.

forms related to resident appointments with outside providers. Assure that all information related to medical appointments is properly disseminated to appropriate caseworker.
ADDITIONAL RESPONSIBILITIES:

Role Summary
At Freedom Village, Residential Aides play a critical role in supporting the safety, stability, and well-being of tenants-survivors of domestic violence and individuals with disabilities, including the frail elderly. Working across all shifts, Residential Aides monitor the building, support front desk operations, respond to tenant needs, and uphold community policies. They foster a respectful, trauma-informed environment that promotes independence, healing, and housing retention. As part of a multidisciplinary team, Residential Aides are essential to creating a secure and welcoming space for all residents of Freedom Village.
Responsibilities
Provide coverage at the Reception Desk in monitoring entry and exit of staff, residents and visitors; answering and directing, as appropriate, telephone calls; and completing other administrative duties as requested by supervisors;
Maintain a safe environment by conducting regular rounds, ensuring the security of the facility, and intervening in conflicts between residents.
Ensure timely and appropriate completion of all log and incident documentation required by supervisors in recording security and safety issues/incidents;
Document all residents, staff and visitors entering and exiting the building; complete census forms, report all incidents, and ensures all events are logged in the Facility Logbook;
Ensure the lobby and front of the building is always cleared;
Prevent loitering in all areas within and surrounding the building;
