Barrier Free Living Apartments (BFLA) is a supportive housing program. BFLA's program is physically designed and equipped to serve people with physical disabilities. The facility is located in the South Bronx. Role Summary
The Resident Aides are a crucial part of the team at Barrier Free Living Apartments, staffing the reception desks. They are vital to ensuring a safe environment for building tenants and program staff. Reception/Front Desk provide crucial assistance to residents by assisting them with connecting to clinical and family center staff. Resident Aides work closely with clinical, family center, and building staff in the regular course of their work.
Responsibilities

Responsibilities:
Ensures all residents are evaluated prior to residency as required by individual state regulations or with change of condition to ensure services are delivered. Presents evaluation to Director, Health and Wellness, Nurse, or Executive Director for approval.
Communicates with residents, family, and staff on an as needed basis to meet resident needs. Participates in care conferences with residents and family members to discuss resident needs and appropriate service delivery in association with service plan updates.
Responsible for screening, hiring, training, and supervising all Care Partner staff and activity staff in the Specified Care Area. Assures that medications are properly stored, medication carts/cupboards are cleaned and medication room is organized according to standards.
Observes residents' health statuses, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment, or significant incidences to Director, Health and Wellness in a timely manner. Reviews capabilities of resident who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation).
Conducts pre-move-in screening of prospective residents as needed and according to state requirements. Assures that all required documentation including service plans and nursing assessments is completed.
Performs ongoing assessments of resident physical and psycho-social needs and coordinates with nurse to assure quality proactive care as allowed by state regulation.
Communicates with families regarding residents' personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments.
Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families.
Observes residents, records significant reactions, and notifies Executive Director, Director, Health and Wellness, physician, and pharmacist (as applicable) of resident's condition and reaction to drugs, treatments, and significant incidents.
Responds promptly to all emergency calls from residents, including residents residing in independent section of community.
Performs daily, weekly, and monthly safety and infection control checks of the home and grounds, assisting the Executive Director and the Director, Health and Wellness for resident health and safety purposes.
Coordinates monthly staff schedules, arranges replacement staffing when necessary and maintains clear records of all changes, requires and irregularities.
Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through.
Participates in the maintenance of resident records including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately.
Shares on call duties as required.
Participates in community functions including family socials and open houses. Communicates with community professionals.
Provides guidance for the staff in provision of the Activity Program.
Responsible for inventory control and direction to staff in maintaining budget.
Responsible for oversight and compliance with the communities CQI (Continuous Quality Improvement) Program and takes steps to assure that services delivered are completed as designed. Audits medication administration by staff, documentation, resident weights, and reassessments to assure compliance.
Responsible for all local, state, and federal rules and regulations as they apply to services delivered in the Specified Care area. This involves staff selections, training and oversight, resident care and documentation, and compliance with HIPAA, OSHA, Blood Borne Pathogen Guidelines, and any other rule that applies to the work we do.
Ensures they and any employees reporting to them work in a safe manner and that unsafe actions are corrected. Compliance with Safety Committee Standards is required. Should workplace injuries occur, when required, the Department Heads and Directors assist the Executive Director with the investigation, return to work, and management of the injured worker communication with care provider and the management company.
Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions which assist others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors. Has working knowledge of the organization.

Required Skills and Experience:

Essential Job Duties - GI Focus:
Patient Assessment and Diagnosis: Nurse Practitioners and Physician Assistants (NPs/PAs) will partner with CMO to perform comprehensive GI-focused health assessments, including detailed medical histories, physical examinations, and diagnostic evaluations relevant to gastrointestinal disorders. NPs/PAs will formulate differential diagnoses and develop appropriate treatment plans in collaboration with supervising physicians.
Treatment and Management: NPs/PAs will collaborate with GI CMO to provide specialty care services, including prescribing GI-specific medications, ordering and interpreting GI-related diagnostic tests (e.g., colonoscopies, endoscopies, liver function tests), and implementing therapeutic interventions. They will manage chronic GI conditions, monitor patients' progress, and adjust treatment plans to optimize digestive health outcomes and patient quality of life.
Health Promotion and Disease Prevention: NPs/PAs will educate patients and families about GI health promotion strategies, disease prevention, dietary and lifestyle modifications, and long-term management of chronic GI conditions. They will emphasize preventive care, including colorectal cancer screenings, hepatitis vaccinations, and liver health monitoring.
Collaborative Care: NPs/PAs will work closely with interdisciplinary healthcare teams, including gastroenterologists, hepatologists, primary care providers, dietitians, and nursing staff, to coordinate comprehensive patient care. They will consult with specialists, refer patients appropriately within the GI specialty or to external providers, and ensure continuity of care across inpatient and outpatient settings.

Documentation of Medication Administration: Assure that all medication administration is documented by the individual assisting with administration and follows all applicable regulations. In the event of resident absence or resident refusal of medications, assure that this absence or refusal is documented in compliance with all applicable regulations.
Medication Supply: Assure that there is an adequate supply of medications on hand for each resident. Specifically, in the event less than 3 day supply of medication, order refill, send in new prescription, or refer problem immediately to the assigned caseworker.
Medication Changes: Changes are made only when there is a prescribing practitioner's written order, or when there is a

If there is a medication with a dosage change, but no new prescription (for example, the doctor changes the order from 2 pills twice a day, to one pill twice a day), the MAR is changed to reflect the change and assure that the prescription container is tagged with the change.
Incontinence Supplies: Assist residents with monitoring and management of supplies used to assist with incontinence issues. Work closely with outside providers, insurance companies, pharmacy, Fleming House Social Service and PCA staff to ensure that residents are receiving proper amounts of supplies at appropriate times.

forms related to resident appointments with outside providers. Assure that all information related to medical appointments is properly disseminated to appropriate caseworker.

Conduct intake on residents when necessary.
Maintain all daily weekly, monthly Log Books, and any other required documentation.
Sign residents in and out of the house according to curfew restrictions.
Conduct random drug and alcohol tests.
Maintain inventory of toiletries, cleaning supplies, food supplies, and kitchen supplies.
Shop for supplies when necessary.
Prepare resident meals when necessary.
Assist with creating and assigning resident work detail, making sure the house is presentable at all times, i.e., kitchen crew, in-house cleaning assignments, lawn maintenance, snow removal, etc.
Transport residents to meetings, DSS, medical appointment, etc. when needed.
Participate in weekly House Meetings and Morning Meetings when on duty.
Make sure all doors are locked after Curfew.
Conduct and record monthly fire drills as per the Company policy
Resident medication management and distribution.
Be available to work all shifts to ensure proper coverage per contract and policy standards.
