• Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves.
• Keep an accurate record of food intake and output, as directed.
• Maintain established housekeeping standards within assigned duty areas.
• Assist nursing staff with the basics of aseptic and sterile techniques to avoid infection of residents and self contamination of equipment and supplies.
• Be alert for proper ventilation, temperature, light and noise control.
• Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed.
• Promptly answer call lights and assist with other resident needs.
• Utilize proper body mechanics when lifting or transporting residents.

Job Requirements:

Responsibilities/General Duties (Including but not limited to):
Assists patients with personal hygiene, including shower, tub or bed baths and mouth, skin and hair care - all activities of daily living that are required.
Assists patients in the use of toilet facilities, including bedpans, incontinent care etc.
Assists patients in and out of bed, including the use of mechanical lifting equipment, when applicable
Assists patients with walking, including the use of walkers and wheelchairs, when applicable
Assists patients with self-administration of medications
Assists with simple meal preparation and feeding, when required
Measures and records fluid intake and output
Takes and records blood pressure, pulse and respiration when required
Maintains and uses EMR according to agency guidelines

Documents services delivered in accordance with Hospice medical records policies and procedures, and submit documentation in a timely fashion i.e.: weekly
