Offering emotional support and actively listening to residents which will help them develop trust and achieve independence in daily living;
Maintaining the confidentiality of all residents and their care to ensure resident rights and privacy are protected at all times;
Reporting and documenting resident and/or visitor incidents, accidents, complaints or allegations according to the facility's policy and procedure using the designated occurrence reporting system.
Providing medical record documentation in accordance with facility policy as indicated by:
Taking and monitoring vital signs, intake and outputs, weights, and meal consumptions as assigned by staff nurse;
Complying with documentation requirements for functional maintenance plans, 24 hour reports, CNA flow sheets, monthly intake logs, BM flow records, behavior management records, toileting plans, shower lists, skin care audits, cleaning schedules, protective device usage and other records as assigned.
Complying with safety requirements, infection control measures, and maintaining a clean work environment in accordance with facility policy as indicated by:
Using protective devices (side rails, Geri chairs and poseys) appropriately and under the direction of the staff nurse;
Abiding by all emergency protocols for crash carts, resident codes, fire safety, missing residents, tornado preparation, violent residents, Wander guard alarms, door alarms, and other facility policies as assigned;
Reporting any forms of abuse witnessed or aware of to the nurse manager immediately;
Following infection control and universal precautions procedures;
Following periodic cleaning schedules for equipment and returning equipment to proper area after use, and
Ensuring the nursing service work areas are maintained in a clean and sanitary manner.
Performing other duties as assigned by staff nurse or designee.
Qualifications
Minimum Qualification/Requirements:
Must possess specific educational and experience requirements such as:
A high school diploma or GED, unless working under a student permit.
Be certified by the Indiana State Department of Health (ISDH) as a C.N.A. in good standing. CNAs transferring from another state may work for 120 days while awaiting their Indiana certification. Graduating CNA students, not yet certified, may also work for 120 days while awaiting their Indiana certification.
Have basic computer skills along with knowledge of such programs as Excel, Word, etc.
Have a thorough understanding of the principles of safe effective nursing practices within the State of Indiana.
Must possess interpersonal skills such as:
Assisting licensed nurse with procedures within the scope of practice for nurseaide;
The ability to follow written and verbal instructions, function independently and make independent decisions when circumstances warrant such action;
Personal integrity and the ability to deal tactfully with residents, family members, visitors, government agencies and personnel;

About the role:
The PRN hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations, and documenting observations and care performed.
The PRN hospice aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
Job Responsibilities:
1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
2. Providing patient and family with positive communication techniques, within the realm of ethical and respectful care, including confidentiality.
3. Meeting the safety needs of the patient and using equipment safely and properly (foot stools, side rails, O2, etc.)
4. Giving personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.
5. Assisting in dressing and undressing patients as assigned.
6. Planning and preparing nutritious meals, including shopping, as assigned.
7. Assisting in feeding the patient as assigned.
8. Taking and recording oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency.
9. Providing proper care and observation of the patient's skin to prevent the breakdown of tissue over bony prominence.
10. Assessing and reporting on the patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
11. Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
12. Provide a normal range of motion and transfers/positioning as directed.
13. Assistance with self-administered medications as allowed by state regulations, such as "reminders."
14. Offering and assisting with bedpans and urinals. Assisting as assigned with light laundry needs.
15. Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency.
16. Providing respite for patient's/family/caregiver when on-site, as appropriate.
17. Keeping the patient's living area clean and orderly, as assigned.
18. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
19. Participates in quality assessment performance improvement teams and activities.
20. Respectful of patient and family/caregiver environment and patient's personal needs.

HOW YOU'LL WORK:
You'll perform personal care needs for your patient and attend to their requests promptly. You will effectively communicate the patient's condition and significant changes to the assigned supervisory nurse and inform the caregiver of any issues. You'll be responsible for all practices and duties within the Hospice Aide scope of practice as outlined by the state.
MAJOR AREAS OF RESPONSIBILITY:
Patient Care: Provide care that includes baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Assist in dressing and undressing patients as assigned. Assist in meal prep and feeding patients as assigned.
Extended Services: Assist in ambulation and exercise as instructed by the primary care nurse or therapist. Perform range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency.
Communication: Maintain good relationships with the patient, the patient's family, and staff. Reports any changes to the patient's medical and living conditions to the supervisor or the primary nurse.
Minimize Patient Risk: Meet safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.). Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence.
Policies: Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
Operations: Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude.
Understanding of the hospice philosophy, comfortable providing specialized care to the terminally ill and secure with the issues of death/dying.
Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS:
Meets the training requirements of the State and has successfully completed a state established or other training program that meets the State's requirements of a competency evaluation program, or state licensure program.
One year of experience is preferred, preferably in Hospice care.
Valid driver's license and auto insurance in your name as a driver

Assist residents in dressing appropriately, encouraging usual and appropriate clothing.
Make residents' beds and change linens as needed, clean up room and closet, picking up unnecessary articles and arranging room neatly.
Feed residents requiring total or partial assist with meals, set up trays for residents needing assistance, and pass and assist with snacks.
Answer calls light promptly and ensure lights are always within reach.
Assist residents with bedpan, urinal, and bedside commode or to bathroom as needed and/or as directed. Change incontinent brief or diaper every two hours or more frequently if needed.
Take and give notice in writing, vital signs (temp, pulse, respiration & blood pressure), heights and weights as requested, report abnormal or unusual signs to charge nurse. Report skin breakdown, pressure sores, changes in residents' physical/mental condition to nurse immediately.
Measure intake and output on all residents for whom it has been ordered including emptying urine from indwelling catheters or other urine collection devices near end of shift and assist in collection of specimens.
Pass water to residents and ensure it is within reach. Collect water pitchers for cleaning per schedule.
Clean wheelchairs, walkers, commodes, bedpans and urinals per schedule.
Report unusual color or odor of excretes (urine, drainage, stool or sputum) before discarding it.
Assist in the transport, transfer, ambulation, frequent positioning and providing ROM to residents. Use measures that ensure resident and associate safety. Request assistance when lifting, turning and moving residents, using lifting devices when needed. Transport to activities, meals and appointments as requested.
Help maintain personal dignity of each resident by ensuring privacy and treating them with respect. Understand and follow Resident Bill of Rights.
Apply safety devices as directed by the nurse, check safety devices every thirty minutes and release every two hours.
Ensure residents who are unable to call for help are checked frequently.
Ensure rooms are ready for new admissions.
Assist in admission, transfer and discharge of residents including ensuring rooms are ready for admit and assist resident with belongings upon discharge.
Greet newly admitted residents on admission and escort them to their room as necessary.
Complete NurseAide flow sheet during shift and according to instructions using only authorized abbreviations.
Document care given in CareTracker.
Identify resident before providing care.
Follow universal precautions and isolation procedures for infection control. Wash hands at beginning of shift, between residents, before passing trays and after assisting with elimination-even if wearing gloves.
Inventory and mark resident's personal possessions as instructed.
Report occupational exposures to blood, body fluids, infectious materials and hazardous chemicals immediately.
Maintain all service areas in clean and orderly condition and keep halls clear of equipment.
Report all accidents or incidents observed immediately.
Report suspected abuse immediately.
Perform other duties as assigned
