(CNA - Certified Nursing Assistant):
Direct and monitor the personal care duties carried out by Nursing Assistants of an assigned team.
Report to Charge Nurse unusual symptoms, daily needs and progress of residents.
Notify Charge Nurse of changes in residents' conditions or needs
Follow the Profile of Care for each resident and monitor the performance of the Nursing Assistants in carrying out the appropriate aspects of the care plan.
Responsible for responding immediately to residents' requests.
Assist residents with the following Activities of Daily Living (ADL), according to their plan of care and individual needs.
Qualifications
(CNA - Certified Nursing Assistant):
Seventeen (17) years of age required; high school diploma or equivalent preferred
Employment Certificate/Permit required for 17 years of age.
Registration in the State Nurse Aide Registry (CNA) is required. Active on the State Nursing Aide Registry with no findings or convictions of resident abuse, mistreatment or misappropriation of property.
Training and work experience in a geriatric setting preferred.
Ability to supervise and work well with other personnel
Ability to communicate well verbally and in writing
Caring and respectful attitude towards residents
Physical stamina for constant activity required.
Ability to perform tasks to established standards of excellence required.

Participate in the daily care of residents such as checking schedules for resident appointments completing personal inventory upon admission, reporting care given and observations.
Provide personal care to residents such as bathing, dressing, passes nourishment, collecting specimens of urine, stool, and sputum, performing post-mortem care.
Assist with skincare and repositioning such as reporting skin breakdown, applying lotion, turning residents side to side, using mechanical lifts, toileting residents.
Maintain an orderly and clean environment.
Use safe procedures to prevent injuries to residents, self, and others.
Complete required documentation such as intake/output forms, vitals and weight.
Interact with residents, families, and other staff.
Qualifications:
High School diploma or equivalent
Valid NYS CNA Certification
The willingness to provide excellent age-appropriate care to residents
Good communication and customer service skills
Ability to handle unpleasant sights and odors.
Must be team-oriented

Functions

· Observes residents and notes physical condition, attitude, reactions, appetites, and reports changes or unusual findings to the Charge Nurse responsible for reporting for reporting resident complaints to the Nurse or Supervisors.
· Before getting off duty, signs the aide book kept at the nurses station
· Attends regularly scheduled in-service classes as required by HOME and NYSAOH regulations.
· Cleans equipment (carbonized beds, stands, etc.) as directed by the nurse. Keeps the utility and service rooms in order.
· Maintains professional attitude and appearance
· Provides assistance with resident care, where applicable.
· Follows organization and departmental policies and procedures.
· Follows assigned safety, security, and fire procedures.
· Reports and accidents or dangerous situations immediately.
· Maintains confidentiality of organizational and resident information.
