    Under the supervision of the Transportation Coordinator, the Medical Transport is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies. Location: Central Valley PACE Center in Merced, Ca. Schedule: Monday - Friday. This position requires availability between the hours of 8:00am - 5:00pm. Compensation: $25.22 - $26.48 an hour Duties and Responsibilities Effectively communicates with participants, families, staff and other agencies and departments. Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures wheelchair lifts and tie down equipment are properly operated and participants are delivered to appointments in a timely manner. Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes. Assists passengers on and off the van in a safe and courteous manner. Will work with medical equipment and manage other emergencies that may arise. Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members. Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse. Inspects van and maintains it in a clean, orderly and safe condition. The driver must exhibit exceptional customer service to patients, families, case managers, facilities, physicians, and nurses. Completes driver log each time the vehicle is used. Keep an accurate record of all facility transports, mileage and times. Provides facility administrator with completed logs. Inspects the company vehicle monthly using the Vehicle Maintenance Log. Reports all maintenance or vehicle needs to the facility administrator timely and follow up as appropriate. Assists with maintaining all maintenance records and other records regarding the company vehicle. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties Completes documentation as required. Coordination of 24 hour care delivery. Other projects and duties as assigned. Physical Demands Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff. Must have vision, with or without lenses, adequate to read memos, a computer screen, personnel forms and clinical and administrative documents. Must have high manual dexterity. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling. Work Environment Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment. Subject to unpleasant odors. Exposed to outdoor elements of heat, cold and rain when transporting patients. The noise level is usually quiet to moderate, but may at times be noisy and crowded. Education/Experience Requirements Minimum Qualifications Valid CA Driver's License, acceptable driving record, and vehicle insurance. Must be able to work effectively in a team environment and operate handicap accessible vehicles. Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently Education/Experience High School Diploma, GED, or equivalent required, or Class A in lieu of education Minimum two (2) years' experience transporting individuals with special mobility needs, school bus driver, or transit/intercity bus driver required. Current BLS CPR Card certified by the American Heart Association Certified Nurse Assistant preferred. Prior personal care assistance experience, or ability to pass internal competencies, preferred.
