    Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies. Location: Stoddard Rd Modesto, CA 95356 Schedule: Monday - Friday. This position requires availability between the hours of 9:00am-6:00pm. Compensation: $21.79 - $22.88 an hour. Duties and Responsibilities Effectively communicates with participants, families, staff and other agencies and departments. Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures wheelchair lifts and tie down equipment are properly operated and participants are delivered to appointments in a timely manner. Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes. Assists passengers on and off the van in a safe and courteous manner. Will work with medical equipment and manage other emergencies that may arise. Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members. Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse. Assists with maintaining all maintenance records and other records regarding the company vehicle. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties Completes documentation as required. Coordination of 24 hour care delivery. Other projects and duties as assigned. Physical Demands Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
