    Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies. Location: Stoddard Rd Modesto, CA 95356 Schedule: Monday - Sunday- Per Diem (as needed). This position requires availability between the hours of 6:00am-6:00pm. Work hours will vary based on need, with shifts ranging from 2 to 8 hours per day when called upon, not exceeding 15 hours per week. Compensation: $21.79 - $22.88 an hour. Duties and Responsibilities Effectively communicates with participants, families, staff and other agencies and departments. Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures wheelchair lifts and tie down equipment are properly operated and participants are delivered to appointments in a timely manner. Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes. Assists passengers on and off the van in a safe and courteous manner. Will work with medical equipment and manage other emergencies that may arise. Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members. Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse. Inspects van and maintains it in a clean, orderly and safe condition. The driver must exhibit exceptional customer service to patients, families, case managers, facilities, physicians, and nurses. Completes driver log each time the vehicle is used. Keep an accurate record of all facility transports, mileage and times. Provides facility administrator with completed logs. Inspects the company vehicle monthly using the Vehicle Maintenance Log. Reports all maintenance or vehicle needs to the facility administrator timely and follow up as appropriate. Assists with maintaining all maintenance records and other records regarding the company vehicle. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties Completes documentation as required. Coordination of 24 hour care delivery. Other projects and duties as assigned. Physical Demands Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff. Must have vision, with or without lenses, adequate to read memos, a computer screen, personnel forms and clinical and administrative documents. Must have high manual dexterity. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling. Work Environment Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment. Subject to unpleasant odors. Exposed to outdoor elements of heat, cold and rain when transporting patients. The noise level is usually quiet to moderate, but may at times be noisy and crowded. Education/Experience Requirements Minimum Qualifications Valid CA Driver's License, acceptable driving record, and vehicle insurance. Must be able to work effectively in a team environment and operate handicap accessible vehicles. Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently. Education/Experience High School Diploma, GED, or equivalent required, or Class A in lieu of education. Minimum six (6) months experience transporting individuals with special mobility needs required. Current BLS CPR Card certified by the American Heart Association Certified Nurse Assistant preferred. Prior personal care assistance experience, or ability to pass internal competencies, preferred.

Also provides or assists with grooming and hygiene of residents (i.e. shaving, oral hygiene, cleaning, trimming finger/toenails) in a safe and thorough manner on a daily basis. • Makes beds, straightens rooms, and keeps resident care areas in an organized manner. • Assists residents in dressing when needed and demonstrates concern for individual privacy while providing resident care. • Provides safe resident transfers involving bed, wheelchair, chair and/or shower chair. • Demonstrates ability to assist with toileting in a safe and thorough manner, as indicated in resident's plan of care. • Promotes independence in resident care as appropriate and demonstrates awareness of safety for both residents and the individuals providing care. • Takes and records temperature, pulse, blood pressure, and respirations as required. • Assists residents with simple range of motion or other exercises as directed by nurse or therapist and ambulates residents safely according to his/her endurance levels. • Applies medical or adaptive devices as ordered by the appropriate supervising nurse. • Makes rounds to ensure residents' safety, comfort, medical attention every hour or as required by the patient. • Collects specimens as directed. • Completes the weighing of each resident as required. • Sets up meal tray and assists with feeding as necessary. Assesses for choking and responds appropriately. • Participates in development of resident's plan of care and follows current plan of care. • Promptly reports a change in conditions of residents. • Demonstrates knowledge of standard precautions and proper use of gloves, gowns, masks, eye protection and linen handling. Qualifications Education: High school graduation or G.E.D., preferred. Possession of a valid California Certified Nurse Assistant certification. Proficient ability to communicate orally and in writing. Training and Experience: Six months experience in providing personal care in a long term care facility preferred. Must be able to lift up and over 100 pounds with assistance according to resident's abilities. Must be able to move intermittently during working hours. Must be able to stand and walk for extended periods of time. Must be able to bend, squat, push, stoop and pull as needed especially in fulfilling resident care activities. Must be able to cope with the mental and emotional stress of the position. ENVIRONMENTAL CONDITIONS: Inside work, normal temperatures, some noise, occasional fumes/odors, chemical exposure and potential exposure to bloodborne pathogens. This job cannot be performed without exposure to the stresses associated with an intimate, 24 hour skilled care environment that delivers care and services primarily to disabled and cognitively impaired residents in an aging population. Examples of these stresses include, but are not limited to: emergency health or safety response, weekend and holiday duty, unusual or impaired behavior by residents, family reactions to having a loved one in a nursing home, death and dying, oversight of state surveyors, ombudsmen and federal officials, presence of consultants and attorneys, and variable involvement of medical staff. Qualifications QUALIFICATIONS/REQUIREMENTS: Education: Sufficient education to demonstrate functional literacy License: Current CNA certification issued by the local Department of Health Services or proof of successful completion of an approved OBRA certification program from another state to submit for certification. No findings in the State Nurse Aide Registry concerning physical, mental and/or verbal abuse, neglect, mistreatment of residents or misappropriation of resident's property. (The state registry must be the state issuing the certification). Work Experience: Prior experience giving personal care to impaired individuals in a health care setting preferred. Language Skills: Must be able to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language. Mathematical Skills: Must be able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to perform these operations using units of weight measurement, and volume. Communication Skills: Must have exceptional communication and customer service skills, and be empathetic. Ability to effectively communicate with patients, families, responsible parties, staff and outside resources and agencies.
