Position Summary

Primary Responsibilities
Resident Services
• Carry out all duties properly and effectively as assigned by the Wellness Director and/or the Executive Director. Supervise the caregiver staff on duty in providing quality care and meeting the needs of residents in accordance with the Oaks Senior Living philosophy and policies.
• Adhere to each resident's Negotiated Service Plan and notify the Wellness Director, MOD, and/or the Executive Director of any changes in resident condition.
• Answer and respond immediately to resident calls.
• Seek assistance and advice from the Executive Director or the Wellness Director immediately if you have any doubts or questions when assisting with medications or treatments before you perform the assigned task.
• Assist with serving meals and supervise caregiver staff in the dining room. Ensure that dining room is cleaned after each meal service.
• Control the spread of infection by following standard precautions. Dispose of infectious and potentially infectious waste according to residence policy and procedures.
• Establish and maintain a good relationship with residents and their families.
• Respond to resident emergencies following the proper procedures.
• Assist in developing and maintaining a schedule for the caregiver staff that sufficiently meets the needs of residents and ensures proper staffing.
• Respond to on-the-job injuries in accordance with the Oaks Senior Living policies and procedures.
• Complete 16 hours of continuing education units (CEU) biennially specific to CMA's as outlined by the Alliant GMCF Registry. In addition 16 hours of CEU's must be completed annually.
• Lead by example, encourage teamwork, and promote the Oaks Senior Living philosophy. • Provide an "open door" to employees, addressing any concerns or grievances they may have.
• Maintain CPR & First Aid certification.
Medication Administration
• Follow the six 'rights" of medication assistance (right resident, right drug, right dosage, right time, right route, right record/documentation) each and every time assistance with a medication occurs. Check each of the six "rights" of medication assistance three different times: when the medication is taken out of the medication cart, immediately before assisting with the medication, and immediately after assisting with the medication
• Administer medications to residents according to their Medication Observation Record (MOR) and protect resident privacy. Provide residents with any treatments required as part of their Resident Care Plan as designated. Document clearly and accurately on the Medication Observation Record (MOR) after a medication is taken/given, refused, held, discontinued, changed, disposed of, returned, following the Oaks Senior Livening policy.
• Respect the residents' right to refuse medication/treatment. Protect the residents' safety with medications. Explain to the resident what medication is being given and why, prior to assisting with medication.
• Observe the resident for any adverse reaction to a medication and report it to the Wellness Director and/or the Executive Director immediately. Report all medication errors honestly and immediately. Communications
• Attend all regular staff meetings and required in-service training sessions.
• Effectively communicate residents' needs with caregiver staff at change of shift according to the Oaks Senior Living policies and procedures. Effectively communicate to the Wellness Director and/or the Executive Director any changes noted in a resident's condition or behavior, adverse reaction to a medication, any resident or family member concerns, complaints, refusal of a medication, any errors noted on the Medication Observation Record (MOR).
• Ensure required paperwork for providing resident care is completed and up-to-date. Obtain, document, and promptly report vital signs and clinical data (temperature, pulse, respirations, blood pressure, pulse oximetry, height, weight, intake/output, blood glucose) and any deviations or changes in condition (e.g., diabetic or uremic coma, insulin shock, GI hemorrhage) to the RN or provider.

Provide care for patients with infections and communicable diseases, including those in isolation; care for patients receiving oxygen via cannula, mask, or venti‑mask; support patients receiving chest therapy and postural drainage.
Provide catheter care for patients with condom catheters and indwelling Foley catheters, following infection‑control protocols.
Administer basic life support measures and immediately notify the RN/provider of emergencies.
Verbally contribute information for Nursing Care Plans, document vital signs and observations in the medical record, and assist with completion of admission histories.
Promote patient independence by supervising and encouraging self‑care activities such as eating, dressing, mobility, and application of prosthetic devices and braces.

Definition:
The HomeCare Aide performs assigned functions of personal care to the patient in the home under the direction, instruction and supervision of the Registered Professional Nurse.
Line of Authority:
Registered Nurse; DON; Administrator/Director of Services
Qualifications:
Completion of the ninth grade in school
Documentation of successful completion of a training program consisting of at least 75 total hours, including at least 16 hours of classroom instruction and at least 16 hours of supervised practical training meeting the content requirements of the Federal Register, August 14, 1989
Successful completion of the written and on-site HomeCare Aide Competency Evaluation prior to performing patient care independently.
Performance Requirements:
Able to be on feet 7-8 hours per day.
Able to lift 70-80 lbs on a regular basis.
Able to bend, stoop, squat and twist frequently in a day.
Able to see and hear adequately enough to respond to visual and auditory requests from patients.
Able to speak clearly enough to adequately communicate with hearing impaired patients.
Upper extremity mobility which does not impede fine motor coordination and manual dexterity.
Able to read, write and follow written directions.
Must have reliable personal transportation.
Specific Responsibilities:
Renders personal care to patients under the direction and supervision of the Registered Nurse. Personal care may include:
Bathing, oral hygiene, care of the skin, hair and dressing.
Assistance with elimination (use of bedside commode, bedpan or in getting to and from the bathroom; routine catheter care and colostomy care, not including irrigation).
Assistance with repositioning, transfers and/or ambulation including the use of equipment such as lifts, wheelchairs, walkers, canes, etc.
Routine care of prosthetic and orthotic devices.
Dressing changes which do not require the skills of a licensed nurse. FLORIDA SPECIFIC: Reinforcement of a dressing only. No dressing changes.
Assistance with medications which are ordinarily self-administered and do not require the skills of a licensed nurse to be provided safely and effectively. FLORIDA SPECIFIC: Supervision of self-administered medication in the home limited to:
obtaining the medication container from the storage area
preparing necessary items such as juice, water, cups or spoons to assist in self-administration
reminding patient that it is time to take mediation
observation of self-administration
Assistance with prescribed exercises after adequate instruction and supervision has been given by appropriate professional personnel.
Assists in maintaining a hygienic environment which may include:
Prepares meals, feeds the patient and cleans utensils used in carrying out these duties;
Provides light cleaning of the patient's immediate environment to keep it safe and in order;
Launders bed linen and needed clothing when the patient is unable to do these things;
Participates in ongoing patient care coordination with all disciplines.
Other duties as assigned by the Registered Nurse.
