9. Provides and reinforces behavior consistent with the Residents' dignity.
10. Reports observations and other pertinent Nursing Rehab/Restorative Care information to the Interdisciplinary Rehab/Restorative Team. Participates in the identification of staff and Resident education needs related to the Nursing Rehab/Restorative Care Program and services.
11. Takes Residents' vital signs; positions and drapes Resident for examination or treatment; transports patients to other areas of the facility in a wheelchair or gurney.

POSITION SUMMARY : The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.
ESSENTIAL DUTIES AND RESPONSIBILITIES : Every effort has been made to identify the essential functions of this position. Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).
Greet residents and escort them to their room.
Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate.
Inventory and mark the resident's personal possessions as instructed.
Transport residents to new rooms or to the receiving area and assist with loading/unloading residents from vehicles as necessary.
Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
Report all complaints and grievances made by the resident.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to your supervisor.
Participate in and receive the nursing report upon reporting for duty.
Follow established policies concerning exposure to blood/body fluids.
Assist residents with daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.).
Assist residents with bath functions (i.e., bedbath, tub or shower bath, etc.) as directed.
Assist residents with personal care.
Keep residents dry (i.e., change gown, clothing, linen, etc., when it becomes wet or soiled).
Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident.
Assist resident with bowel and bladder functions (i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.).
Maintain intake and output records as instructed.
Perform restorative and rehabilitative procedures as instructed.
Change dressings, bandages, binders, etc., as instructed.
Provide eye and ear care (i.e., warm/cold compresses, eye/ear instillation, cleaning eyeglasses/hearing aides, etc.) as instructed.
Answer resident calls promptly.
Ensure that residents who are unable to call for help are checked frequently.
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
Observe and report the presence of pressure areas and skin breakdowns to prevent pressure ulcers (bedsores).
Report injuries of an unknown source, including skin tears.
Turn bedfast residents at least every two (2) hours.
Perform special treatment as instructed.
Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).
Serve food trays. Assist with feeding as indicated (i.e., cutting foods, feeding, assist in dining room supervision, etc.).
Keep residents' water pitchers clean and filled with fresh water (on each shift) and within easy reach of the resident.
Serve between meal and bedtime snacks.
Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.
Wash hands before and after performing any service for the resident.
Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility.
Follow established safety precautions in the performance of all duties.
Keep excess supplies and equipment off the floor. Store in designated areas.
Wash wheelchairs, walkers, etc., as instructed.
Clean, disinfect, and return all resident care equipment to its designated storage area after each use.
Perform routine housekeeping duties (i.e., clean bedrails, overbed table, night stand, etc., that relate to nursing care procedures).
Report all hazardous conditions and equipment to the Nurse Supervisor/Charge Nurse immediately.
Report all safety violations.
Follow established isolation precautions and procedures.
Follow established procedures in the use and disposal of personal protective equipment.
Use only the equipment you have been trained to use; operate all the equipment in a safe manner.
Inform the nurse Supervisor/Charge Nurse of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan.
Maintain the confidentiality of all resident care information.
Ensure that you treat all residents fairly, and with kindness, dignity, and respect.
Ensure that all nursing care is provided in privacy.
Knock before entering the resident's room.
Report all grievances and complaints made by the resident to the Nurse Supervisor/Charge Nurse.
Report all allegations of resident abuse and/or misappropriation of resident property.
Honor the resident's refusal of treatment request. Report such requests to your supervisor.
Performs other duties as may be assigned.

Perform routine inpatient and/or outpatient nursing duties that do not require a license. Performs related duties as required.
**DUTIES & ESSENTIAL JOB FUNCTIONS**
NOTE: The following are duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in this classification.
+ Assists patients with meals, personal hygiene and incontinent care; gives cleansing and retention enemas to patients; assist in catheterization and bladder irrigations.
+ Assists registered nurse and licensed vocational nurses in administering treatment and other nursing tasks as needed; promptly answers call lights promptly and administers to patient comfort and safety by adjusting beds, lights, bed rails, pillows, patients' clothing and bedside tables/equipment; observes and reports any deviation in patients' condition/status; keeps patient areas clean and orderly.
+ Obtains and sets up suction equipment; performs nasal and tracheal suctions; administers oxygen using tents and masks; turns and positions patients; assists patients to and from wheelchairs and gurneys; and assists patients with ambulation and crutch walking.
+ Takes patients' vital signs (temperature, pulse, respiration, and blood pressure); performs routine tests such as urine dip stick, urine pregnancy test, vision and hearing tests and promptly reports findings to supervising personnel; administers simple range of motion exercises; assists in application of traction devices; positions and drapes patients for examination or treatment; transports patients to other areas of the hospital in wheelchair or gurneys.
+ When assigned as a sitter, responsible for but not limited to protecting the patient(s)/resident(s) from harm and assiting in meeting their emotional and physical needs through one-to-one continuous observation. Provide on-going observation and basic patient care. Educate patients on health related topics or discharge information per scope of duties.
+ When assigned to an outpatient setting, prepares examination room, ensuring that it is stocked with the necessary supplies, equipment and instruments for the type of clinic to be held; weighs, takes and documents patients' vital signs; assists physicians or midlevel practitioners during the examinations as directed; collects, labels and takes specimens to the lab; assists in the maintenance of a clean and well supplied clinical area; stocks shelves and monitors expiration of medical supplies and medications.
**QUALIFICATIONS**
Education: High School diploma or GED with additional training a minimum. College graduate preferred.
Minimum Experience: Six months para-professional nursing training as a Hospital Services Trainee with AHS or the equivalent of one year full-time recent experience performing work comparable to a Nursing Assistant in a hospital or medical facility.
Required Licenses/Certifications: BLS - Basic Life Support Certification issued by the American Heart Association. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
Required Licenses/Certifications: CPI -Crisis Prevention Intervention Training (required for all positions at John George Psychiatric Pavilion; and certain positions in the Emergency Department).
Required Licenses/Certifications: Current Nursing Assistant certification (required for positions at Highland and Fairmont Hospitals).
**PAY RANGE**
$34.64 to $41.23 per hour
