* Provides personal care services to patients.
Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.

Chemical Storage Policy
Policy: To keep hazardous cleaning solutions, chemicals, poisons, and substances of similar nature in a safe area to prevent residents and/or visitors from harm.
Procedure:
Cleaning solutions, chemicals, poisons, and substances of similar nature shall be labeled with original label from supplier, and stored in a locked storage area. All liquid chemicals are to be stored below any and all paper products.
If housekeeping staff is cleaning a room the cart of cleaning solutions and chemicals must be within eyesight at all times.
If cleaning cart is not within eyesight, all chemicals must be locked.
Medication First Dose Policy
Policy: To provide safe administration of medications that the resident has not previously had administered to him/her.
Procedure:
The facility shall instruct the UAP in the importance of administering first-time medications.
This is a medication that is not listed on the current MAR for the resident. This will be accomplished by the UAP completing a South Dakota Board of Nursing approved Medication Course and by the origination of this policy.
The licensed nurse will provide education for the first-time medication being given prior to giving the medication, including possible side effects. This information will be shared with the UAP by telephone or in person.
The UAP shall look for any documentation of allergies for the resident who is to receive the first-time given medication. The UAP shall also look for any documentation of side effects or drug interactions for the medication to be given. These issues shall be discussed with the facility nurse prior to the UAP giving the first dose of any new medication to a resident.
The UAP shall document on the back of the MAR when he/she talked to the nurse in regards to first dose with note stating: “
Date/Time First dose instructions for medication name given to staff by nurse name
. When nurse is on campus, she will review order, verify first dose note and sign off on MAR.
The licensed nurse shall delegate the task of administering first-time given medications only to the UAP he/she has determined can safely perform this task.

MAJOR AREAS OF RESPONSIBILITY:
The primary purpose of your job is to provide each of your assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.

Follow established policies concerning exposure to blood/body fluids.
Assist residents with daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.).
Assist residents with bath functions (i.e., whirlpool bath, shower, bed bath, etc.) as directed.
Give backrubs as instructed.
Assist residents with dressing/undressing as necessary.

Assist residents with nail care (i.e., clipping, trimming and cleaning fingers/toenails). (Note: Does not include diabetic residents).
Shave residents as needed.
Keep hair on female resident's clean shaven (i.e., facial hair, under arms, legs, etc.) as instructed.
Keep residents dry (i.e., change gown, clothing, linen, incontinent products, etc., when it becomes wet or soiled).
Keep the call cord/nurses call system within easy reach of the resident.
Change bed linens and keep them tight to avoid wrinkles from forming under the resident.
Make beds either occupied or unoccupied.
Put extra covers on beds as requested.
Position bedfast residents in correct and comfortable position.
Assist resident with bowel and bladder functions (i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.).
Maintain intake and output records as instructed.
Check and report bowel movements and character of stools as instructed.
Collect specimens as instructed (i.e., urine, sputum, stools, etc.).
Assist resident in preparing for activity and social programs (i.e., church, parties, visitors, etc.).
Assist residents in getting ready to transport to/from appointments, activities, social programs, etc., as necessary.
Assist with lifting, turning, moving, positioning, and transporting resident into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Assist residents to walk with or without self-help devices as instructed.
Perform restorative and rehabilitative procedures as instructed.
Provide eye and ear care (i.e., warm/cold compresses, eye/ear instillation, cleaning eyeglasses/hearing aids, etc.) as instructed.
Assist with the application of non-sterile (moist and dry) warm/cold compresses.
Assist in preparing the resident for a physical examination.
Weigh and measure resident as instructed.
Answer resident's call lights promptly and leave call cord with reach of the resident.
Ensure that residents who are unable to call for help are checked frequently.
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
Assist with care of dying resident.
Provide post-mortem care as instructed.
Report any missing resident item to the Charge Nurse
Special Nursing Care Functions:
Observe and report the presence of pressure areas and skin breakdowns of any origin to the Charge Nurse.
Report injuries of an unknown source, including skin tears.
Provide daily indwelling catheter care.
Provide daily perineal care.
Assist with the application of slings, elastic bandages, binders, etc.
Turn bedfast residents at least every two (2) hours.
Observe disoriented residents and re-direct as needed.
Turn all medications found in the resident's room over to the Charge Nurse.
Watch for and report any changes in room temperature, lighting, flooring, etc. to maintenance or charge nurse.
Food Service Functions:
Prepare residents for meals (i.e., wash hands, raise bed, position tables, take to/from dining room, etc.).
Serve food trays to the residents. Assist with feeding as indicated (i.e., cutting foods, assisting with eating, identifying food on the plate, etc.).
Assisting residents with identifying food (i.e., informing resident with sight problem of the food on his/her plate, where it is located, if it is hot or cold, etc.).
Record the resident's food/fluid intake and report any changes in the resident's eating habits.
Keep resident's water pitchers clean and filled with fresh water and within easy reach.
Serve snacks between meals and at bedtime.
Perform after meal care for the residents (i.e., cleaning resident's hands and face, removing trays, taking to the bathroom, brushing teeth, cleaning dentures, etc.).
Checking rooms for food articles (i.e., food that is spoiled, food in properly label container, unauthorized food items, etc.).
Staff Development:
Attend and participate in scheduled in-service training and educational classes to maintain current certification as a Certified NurseAide.
Attend and participate in scheduled orientation programs and activities.
Safety and Sanitation:
Wash hands before and after performing any service for the resident.
Do the following for any resident on a restraint - check every thirty (30) minutes; release every two (2) hours for range of motion exercises and taking to the bathroom; and record the times the restraints were released and for how long.
Participate in appropriate in-service training prior to performing tasks that involve potential exposure to blood/body fluids.
Immediately notify the Charge Nurse of any resident leaving or missing from the facility.
Follow established safety precautions in performance of all duties.
Follow the “Limited Lift” policy.
Keep resident's personal possessions off the floor and properly stored.
Keep excess supplies and/or equipment off the floor and properly stored.
Clean up or report any spills immediately.
Wash wheelchairs, walkers, etc., as instructed.
Clean, disinfect, and return all resident care equipment to its designated storage area after each use.
Perform routine housekeeping duties (i.e., clean over bed table, night stand, call cord, etc., that relates to nursing care procedures).
Before leaving work area for breaks or the end of the day store all tools, equipment and supplies in there appropriate place.
Report all hazardous conditions and equipment to the Charge Nurse immediately.
Report any safety violations.
Follow established smoking regulations and report any violations.
Report any communicable or infectious disease to the Director of Nursing and/or the Infection Control Coordinator.
Follow established isolation precautions and procedures.
Follow established procedures in the use and disposal of personal protective equipment.
Use safety equipment and supplies when lifting or moving a resident (i.e., slides, lifts, gait belts, etc.).
Report any missing/illegible labels off any chemicals to the Director of Nursing.
May be necessary to assist in the evacuation of residents during an emergency.
Duties and Responsibilities:
Record all resident information on computer, flow sheets, charts, etc.
Review Care Plans daily to determine if a change in resident's daily cares have been made.
Report any changes in the resident's condition to the Charge Nurse/Director of Nurses.
Report all accidents and incidents you observe on your shift.
Report any resident abuse immediately to the Administrator, Director of Nursing, Social Worker or Charge Nurse.
Admission, Transfer and Discharge Functions:
Ensure that the resident room is ready for any new resident (i.e., water pitcher, call cord, etc.).
Help greet the resident and escort them to their room.
Introduce resident to his/her roommate, if any, and other residents and staff.
Make resident as comfortable as possible.
Assist resident or their family with storing or packing their personal possessions.
Assist with loading/unloading of a resident from vehicles.
Equipment and Supplies Functions:
Use only equipment you have been trained to use.
Operate all equipment in a safe manner.
Use only the equipment and supplies necessary to do the job, do not be wasteful.
Report defective equipment to the Charge Nurse or to Maintenance.
Inform the Director of Nursing of any equipment and/or supply needs.
Working Conditions:
Works throughout the nursing home in resident rooms, hallways, etc.
Moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, personnel, visitors, government agencies, etc., under all conditions and circumstances.
Is subject to hostile and emotionally upset residents, family member's personnel and visitors.
Communicates with nursing personnel and other department personnel.
Works beyond normal working hours, on weekends and holidays etc.
Is subject to callbacks during emergency conditions such as severe weather, evacuations, etc.
Is subject to injury from falls, burns, faulty equipment, lifting, etc.
Is subject to exposure from dust, disinfectants, smoke, infectious waste, diseases, etc.
May be subject to exposure from hazardous chemicals.
Specific Requirements:
Must be able to read, write, speak and understand the English language.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seeking out and accept new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident's condition.

Medication First Dose Policy
Policy: To provide safe administration of medications that the resident has not previously had administered to him/her.

The facility shall instruct the UAP in the importance of administering first-time medications.
This is a medication that is not listed on the current MAR for the resident. This will be accomplished by the UAP completing a South Dakota Board of Nursing approved Medication Course and by the origination of this policy.
The licensed nurse will provide education for the first-time medication being given prior to giving the medication, including possible side effects. This information will be shared with the UAP by telephone or in person.
The UAP shall look for any documentation of allergies for the resident who is to receive the first-time given medication. The UAP shall also look for any documentation of side effects or drug interactions for the medication to be given. These issues shall be discussed with the facility nurse prior to the UAP giving the first dose of any new medication to a resident.

RESPONSIBLE TO: Charge Nurse, Director of Nursing, and Administrator
QUALIFICATIONS:
Must be a licensed Certified Nurse Assistant in accordance with the laws of the State of Minnesota or be able to obtain licensure. Tuff Memorial Home will provide assistance with obtaining licensure for the right candidate.
Must possess a minimum of an 8th grade education.
Must attend a minimum of 12 hours of in-service meeting per year.
Must have a sincere desire to work with the elderly and possess patience and compassion.
MAJOR AREAS OF RESPONSIBILITY:
The primary purpose of your job is to provide each of your assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.
JOB FUNCTIONS: Every effort has been made to identify the essential functions of this position. However, it in no way states or implies that these are the only duties required by a C.N.A.
Personnel Functions:
Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.
Follow work assignments and/or work schedules in completing and performing your assigned tasks.
Cooperate with inter-department personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
Meet with the Charge Nurse on a regular basis to assist in identifying and correcting problem areas, and/or the improvement of services.
Report all complaints and grievances made by the resident.
Report occupational exposures to blood, body fluids, infectious materials and hazardous chemicals to your supervisor.
Notify the facility when you will be late or absent from work.
If you are going to be absent from work call at least three workers to find your replacement.
Personal Nursing Care Functions:
Participate in and receive the nursing report upon reporting for duty.
Follow established policies concerning exposure to blood/body fluids.
Assist residents with daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.).
Assist residents with bath functions (i.e., whirlpool bath, shower, bed bath, etc.) as directed.
Give backrubs as instructed.
Assist residents with dressing/undressing as necessary.
Assist residents with hair care functions (i.e., combing, brushing, shampooing, etc.).
Assist residents with nail care (i.e., clipping, trimming and cleaning fingers/toenails). (Note: Does not include diabetic residents).
Shave residents as needed.
Keep hair on female resident's clean shaven (i.e., facial hair, under arms, legs, etc.) as instructed.
Keep residents dry (i.e., change gown, clothing, linen, incontinent products, etc., when it becomes wet or soiled).
Keep the call cord/nurses call system within easy reach of the resident.
Change bed linens and keep them tight to avoid wrinkles from forming under the resident.
Make beds either occupied or unoccupied.
Put extra covers on beds as requested.
Position bedfast residents in correct and comfortable position.
Assist resident with bowel and bladder functions (i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.).
Maintain intake and output records as instructed.
Check and report bowel movements and character of stools as instructed.
Collect specimens as instructed (i.e., urine, sputum, stools, etc.).
Assist resident in preparing for activity and social programs (i.e., church, parties, visitors, etc.).
Assist residents in getting ready to transport to/from appointments, activities, social programs, etc., as necessary.
Assist with lifting, turning, moving, positioning, and transporting resident into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Assist residents to walk with or without self-help devices as instructed.
Perform restorative and rehabilitative procedures as instructed.
Provide eye and ear care (i.e., warm/cold compresses, eye/ear instillation, cleaning eyeglasses/hearing aids, etc.) as instructed.
Assist with the application of non-sterile (moist and dry) warm/cold compresses.
Assist in preparing the resident for a physical examination.
Weigh and measure resident as instructed.
Answer resident's call lights promptly and leave call cord with reach of the resident.
Ensure that residents who are unable to call for help are checked frequently.
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
Assist with care of dying resident.
Provide post-mortem care as instructed.
Report any missing resident item to the Charge Nurse
Special Nursing Care Functions:
Observe and report the presence of pressure areas and skin breakdowns of any origin to the Charge Nurse.
Report injuries of an unknown source, including skin tears.
Provide daily indwelling catheter care.
Provide daily perineal care.
Assist with the application of slings, elastic bandages, binders, etc.
Turn bedfast residents at least every two (2) hours.
Observe disoriented residents and re-direct as needed.
Turn all medications found in the resident's room over to the Charge Nurse.
Watch for and report any changes in room temperature, lighting, flooring, etc. to maintenance or charge nurse.
Food Service Functions:
Prepare residents for meals (i.e., wash hands, raise bed, position tables, take to/from dining room, etc.).
Serve food trays to the residents. Assist with feeding as indicated (i.e., cutting foods, assisting with eating, identifying food on the plate, etc.).
Assisting residents with identifying food (i.e., informing resident with sight problem of the food on his/her plate, where it is located, if it is hot or cold, etc.).
Record the resident's food/fluid intake and report any changes in the resident's eating habits.
Keep resident's water pitchers clean and filled with fresh water and within easy reach.
Serve snacks between meals and at bedtime.
Perform after meal care for the residents (i.e., cleaning resident's hands and face, removing trays, taking to the bathroom, brushing teeth, cleaning dentures, etc.).
Checking rooms for food articles (i.e., food that is spoiled, food in properly label container, unauthorized food items, etc.).
Staff Development:
Attend and participate in scheduled in-service training and educational classes to maintain current certification as a Certified NurseAide.
Attend and participate in scheduled orientation programs and activities.
Safety and Sanitation:
Wash hands before and after performing any service for the resident.
Do the following for any resident on a restraint - check every thirty (30) minutes; release every two (2) hours for range of motion exercises and taking to the bathroom; and record the times the restraints were released and for how long.
Participate in appropriate in-service training prior to performing tasks that involve potential exposure to blood/body fluids.
Immediately notify the Charge Nurse of any resident leaving or missing from the facility.
Follow established safety precautions in performance of all duties.
Follow the “Limited Lift” policy.
Keep resident's personal possessions off the floor and properly stored.
Keep excess supplies and/or equipment off the floor and properly stored.
Clean up or report any spills immediately.
Wash wheelchairs, walkers, etc., as instructed.
Clean, disinfect, and return all resident care equipment to its designated storage area after each use.
Perform routine housekeeping duties (i.e., clean over bed table, night stand, call cord, etc., that relates to nursing care procedures).
Before leaving work area for breaks or the end of the day store all tools, equipment and supplies in there appropriate place.
Report all hazardous conditions and equipment to the Charge Nurse immediately.
Report any safety violations.
Follow established smoking regulations and report any violations.
Report any communicable or infectious disease to the Director of Nursing and/or the Infection Control Coordinator.
Follow established isolation precautions and procedures.
Follow established procedures in the use and disposal of personal protective equipment.
Use safety equipment and supplies when lifting or moving a resident (i.e., slides, lifts, gait belts, etc.).
Report any missing/illegible labels off any chemicals to the Director of Nursing.
May be necessary to assist in the evacuation of residents during an emergency.
Duties and Responsibilities:
Record all resident information on computer, flow sheets, charts, etc.
Review Care Plans daily to determine if a change in resident's daily cares have been made.
Report any changes in the resident's condition to the Charge Nurse/Director of Nurses.
Report all accidents and incidents you observe on your shift.
Report any resident abuse immediately to the Administrator, Director of Nursing, Social Worker or Charge Nurse.
Admission, Transfer and Discharge Functions:
Ensure that the resident room is ready for any new resident (i.e., water pitcher, call cord, etc.).
Help greet the resident and escort them to their room.
Introduce resident to his/her roommate, if any, and other residents and staff.
Make resident as comfortable as possible.
Assist resident or their family with storing or packing their personal possessions.
Assist with loading/unloading of a resident from vehicles.
Equipment and Supplies Functions:
Use only equipment you have been trained to use.
Operate all equipment in a safe manner.
Use only the equipment and supplies necessary to do the job, do not be wasteful.
Report defective equipment to the Charge Nurse or to Maintenance.
Inform the Director of Nursing of any equipment and/or supply needs.
Working Conditions:
Works throughout the nursing home in resident rooms, hallways, etc.
Moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, personnel, visitors, government agencies, etc., under all conditions and circumstances.
Is subject to hostile and emotionally upset residents, family member's personnel and visitors.
Communicates with nursing personnel and other department personnel.
Works beyond normal working hours, on weekends and holidays etc.
Is subject to callbacks during emergency conditions such as severe weather, evacuations, etc.
Is subject to injury from falls, burns, faulty equipment, lifting, etc.
Is subject to exposure from dust, disinfectants, smoke, infectious waste, diseases, etc.
May be subject to exposure from hazardous chemicals.
Specific Requirements:
Must be able to read, write, speak and understand the English language.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seeking out and accept new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident's condition.
Must be able to cope with the mental and emotional stress of the position.
Must be in general good health to meet the physical demands of this position.
Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 3 feet and be able to push, pull, move and/or carry 25 pounds a minimum distance of 10 feet.
Must be able to work with ill, disabled, elderly, emotionally upset and at times angry people in this facility.

Assist residents with comfort and anxiety relief: protect resident privacy and maintain confidentiality, keep call light within resident reach, answer call light promptly, provide orientation to room, unit or facility and to other visitors and residents, assist with communication, protect personal possessions, provide diversional activities, give backrubs, prepare hot and cold applications.
Assist residents in promoting safety and environmental cleanliness: make beds, clean and care for equipment, carry out isolation precautions, practice standard precautions, practice medical asepsis and infection control, observe oxygen precautions.
Assist residents with unit management and efficiency: admit, transfer, and discharge residents, transport residents, take specimen to lab, assist with special procedures, telephone answering, documentation and assist in unit record-keeping.
Other duties as assigned.
Keep all knowledge of residents confidential.
Working Conditions:
Must be in good physical and mental condition. Constant standing and walking during work periods. Turning, stooping, bending, stretching, pulling, lifting, moving equipment.
