Assists licensed staff in the provision of basic nursing and therapy care in Skilled Nursing, using the application of basic skills, restorative care, utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served -- adult through geriatric, 65 years or older -- and in collaboration with the multidisciplinary care team. Demonstrates the skill necessary to meet the needs of all residents served in Skilled Nursing through support of the nursing process; collects resident care data, reports findings, assists team in meeting resident care goals. The Certified Nursing Assistant SNF is under the supervision of the Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee.

Experience or specialty training required in vital signs, bed making and resident transfer techniques
Strong computer skills to complete patient data entry and notes

Essential Functions

4. As directed, assists the care team by collecting resident care data at designated and appropriate intervals on all assigned residents. Reports finding to licensed staff.
5. Recognizes and accurately reports relevant resident information to licensed staff.
Reports on all significant changes in resident condition or motivational level.
Notifies charge nurse of feeding problems, reassessment needs.
Notifies PT and charge nurse of any mobility problems, referral or reassessment needs.
6. Utilizes time, equipment, and supplies in an efficient & safe manner.
7. Completes all assigned duties during designated shift. Prioritizes resident care activities based on care needs. Recognizes need for purpose vs. task when caring for residents.
8. Provides personal care assistance to residents, encouraging self care and independence but offering and providing help as needed:

13. Ensures that all aspects of resident care are documented. Documentation is complete, accurate and timely.

Follows procedure when equipment malfunctions.
Ensures use of supplies are accurately documented
15. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the residents served.
16. Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards
17. Adheres to all hospital and departmental policies regarding attendance and punctuality.

Qualifications
EDUCATION AND EXPERIENCE:
A bachelor's degree in social work, psychology, human services, sociology, or a related field is highly desired but not required or two (2) years of experience working with the homeless population and experience in a lead or peer mentoring role is preferred. Working knowledge of all evidence-based practices in ending homelessness is required. CAC/CADC or experience working in alcohol/drug recovery programs preferred. The Community Care Associate - Lead must have a working knowledge of property management, fair housing laws, and credit statements. This position requires demonstrated ability in record keeping, good written and verbal communication skills, disabilities or mental illness, substance abuse and recovery, low income, and the general homeless population. This position requires CPR and First Aid training within ninety (90) days of employment, recertification as necessary, and T.B. testing annually. A valid driver's license and the ability to meet the organization's insurance carrier guidelines are required. You must undergo a criminal background check.
