The Certified Home Health Aide (CHHA) is a member of the Home Health Team, providing direct patient care under the supervision of a case manager, reporting all changes in the patient's condition to the case manager. Essential Responsibilities: Provision of Care: Assisting patients with personal hygiene such as skin, mouth, hair care and bathing Assisting patients in and out of bed and assisting with ambulation. This position is responsible for supporting and coordinating the administrative activities of the nursing unit. May be assigned to cover multiple units.
