SCOPE: Provides direct personal care and services to residents including, but not limited to, safety, comfort and personal hygiene. May provide protection services to residents under the supervision of a Registered Professional Nurse or Licensed Practical Nurse. Duties and RESPONSIBILITIES 1. Bathe, toilet and dress residents. Combs hair, gives mouth and nail care and gives performs general resident grooming.* 2. Assists with meal service including passing of trays and helping to feed residents. Assists residents in the dining room and collects trays.* 3. Distributes nourishments when indicated.* 4. Makes beds and is responsible for complete care of residents' unit and personal possessions.* 5. Assists in the maintaining a clean unit, including, but not limited to, utility room, nurses' station, pantry, day room and shower room.* 6. Take vital signs, weights, measures intake and output, enemas and collects some specimens, test some specimens and report all results to the district nurse. 7. Report any and all changes in residents' conditions to the District Nurse and/or Nurse Manager.* 8. Document all pertinent information pertaining to resident care, including but not limited to, Nurse Aide Accountability Records, Activity of Daily Living Forms, vital signs and weights.* 9. Assists with ambulation, lifting, transferring residents, range of motion exercises, while maintaining proper body alignment and providing a safe environment. Transports residents throughout the facility.* 10. Responsible for giving report of residents' daily activities and nursing services rendered to the District Nurse/Relieving Nurse Aide and/or Nurse Manager before going off duty for break or end of shift.* 11. Follows acceptable infection control practices including standard/universal precautions.* 12. Provides a safe environment for the residents by monitoring day room activities and implementing restorative accommodations as directed by the Nurse Manager or District Nurse. * 13. Attends and completes all mandatory inservices required, including 12 hours of inservice to maintain certification.* 14. Perform other related duties as required.
