ESSENTIAL DUTIES AND RESPONSIBILITIES
Employees are expected to perform essential duties to performance standards. Monitors client activity and conducts facility rounds on all floors by walking through the facility at least hourly.
Monitors and ensures clients take medications as prescribed.
Recognizes antisocial or unusual behaviors and addresses them immediately, positively and in a non-threatening manner. Notifies Supervisor of client problems or issues in accordance with operation procedures.
Listens and responds to client concerns. Documents client behavior and progress and records services rendered to clients in ICL as appropriate. Documents information in staff communication record as appropriate.
Ensures cleanliness of facility and submits work orders for repairs to maintenance and informs Supervisor of the request. Assists residents in cleaning their rooms when necessary.
Maintains safety and security of facility on assigned shift. Conducts scheduled safety drills.
Answers the phone and checks voice mail, greets clients, visitors and vendors. Maintains visitor log.
Escorts client outside for tobacco use during times client is permitted to use tobacco.
Completes Shift Change Report and Daily Shift Report. Completes Incident Reports as needed.
Maintains confidentiality of HIPAA and other confidential and sensitive information.
Maintains the highest level of integrity and professionalism in all aspects of job performance.
Keeps abreast of current trends and “best practices” in the field. Attends and participates in required trainings on topics relevant to the job and to behavioral healthcare.
Performs job responsibilities and maintains all records in accordance with BHP's policy, procedure and protocol, and as appropriate to the job, all regulatory authorities' accreditation/licensure/certification standards, and all applicable laws and regulations.
Works assigned schedule, exhibits regular and punctual attendance and works outside of normal schedule as required.
Specific Shift Responsibilities
In addition to general responsibilities specified above, employees assigned to a particular shift or facility shall perform the following responsibilities:

OTHER DUTIES AND RESPONSIBILITES
Cross trains to work in other residential facilities.
Performs other duties as assigned.
Qualifications: Candidates must possess a high school diploma or GED with training or course work in crisis management and one-year work experience in a residential setting. CPR/BLS required. Basic computer, phone and typing skills are necessary for all positions.

Mental Health Provider Goal: It is the mission and purpose of Quality Correctional Care to assist corrections-involved individuals in their healthcare journey from initial interaction with law enforcement through successful transition back to the community by providing counseling and treatment.
Mental Health Provider Duties/Responsibilities:
Conduct screening, assessments, and recommendations for patients referred by security, healthcare staff, and other community sources.
Develop and recommend treatment plans, conduct essential treatment services such as psychoeducational and transitional groups, and provide suicide prevention interventions and management techniques.
Respond to sick call requests, evaluate medication adherence, and coordinate with prescribing providers for medication evaluations.

Conducts patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret information pertaining to patient/client biopsychosocial functioning. Assessment to include, but not be limited to: developmental and age-specific needs; ability to care for self; ability to care for dependents; ability to meet basic custodial and financial needs; ability to engage in meaningful social and emotional relationships; and ability to comprehend and comply with treatment recommendations. Develops care plan, including discharge plan, incorporating medical plan of care. Participates in identification of cases of abuse, neglect or exploitation and refers such cases to the legally mandated authorities when applicable. Provides crisis intervention, information and referral and linkage to community resources as needed.

Develops a plan of intervention based upon assessment to resolve identified problems and/or to support and enhance biopsychosocial functioning. Through individual, family, or group modalities, employs clinical intervention skills including, but not limited to, treatment planning; continuum of care planning (including discharge planning); supportive counseling; crisis intervention; specialized problem oriented planning and intervention; and educational counseling. Substantial interdisciplinary collaboration, which is a critical piece in holistic care planning for patient and support system.
25% Advanced Population Management
Staff is accountable for coordinating clinical and financial outcomes of an assigned population. Works collaboratively with interdisciplinary staff and external organizations to achieve timely, cost effective management of patient care. Monitors for quality and type of services delivered to patients, ensuring management within established parameters. Functions as a liaison to the payer to manage the post-discharge financial resources and ensure expected treatment process, progress and discharge plan. Monitors and ensures completeness of all assigned patients in areas of treatment plan and timely reports. Help patients coordinate care for the highest quality of life, while managing to the lowest cost. Working with families to ensure access to knowledge and resources.
10% Documentation and Regulatory Compliance
Thoroughly documents assessments and interventions in patient's medical record and information systems in accordance with department standards. Complies with internal and external documentation requirements as related to specific patient/system needs and regulatory requirements.

Maintains awareness of current hospital, community services and appropriate care options that can assist patient/clinical biopsychosocial functioning. Provides information as well as facilitates successful linkages between those services and the patient/client. Performs additional functions as may be appropriate to the position.

ACF Residential Aide
Duties: In this role, you will be responsible for overseeing and providing guidance and direction to BHP residential clients with a history of mental health issues. Listens and responds to client concerns. Documents client behavior and progress and records services rendered to clients in ICL as appropriate. Documents information in staff communication record as appropriate.
Ensures cleanliness of facility and submits work orders for repairs to maintenance and informs Supervisor of the request. Assists residents in cleaning their rooms when necessary.
Maintains safety and security of facility on assigned shift. Conducts scheduled safety drills.
Answers the phone and checks voice mail, greets clients, visitors and vendors. Maintains visitor log.
Escorts client outside for tobacco use during times client is permitted to use tobacco.
Completes Shift Change Report and Daily Shift Report. Completes Incident Reports as needed.
Maintains confidentiality of HIPAA and other confidential and sensitive information.
Maintains the highest level of integrity and professionalism in all aspects of job performance.
Keeps abreast of current trends and “best practices” in the field. Attends and participates in required trainings on topics relevant to the job and to behavioral healthcare.
Performs job responsibilities and maintains all records in accordance with BHP's policy, procedure and protocol, and as appropriate to the job, all regulatory authorities' accreditation/licensure/certification standards, and all applicable laws and regulations.
Works assigned schedule, exhibits regular and punctual attendance and works outside of normal schedule as required.
Specific Shift Responsibilities
In addition to general responsibilities specified above, employees assigned to a particular shift or facility shall perform the following responsibilities:
1st Shift: Conducts inspections, grocery shopping, and prepares meals as needed.
2nd Shift: Assists clients in cleaning rooms and prepares meals as needed.
3rd Shift - River Valley and Altmaier House - Cleans office, staff bathroom, kitchen, and mop floors. Also cleans oven, refrigerator and microwave when needed. Completes paperwork as required.

Job Description
We are seeking a highly skilled LPN or RN to join our home health agency as a Private Duty Nurse for an adult client in Lancaster, Ohio on a part-time basis (Weekends).
Immediate / Starting Schedule = 24 Hours/wk
Required shifts: Saturdays and Sundays (12 hrs both days) w/ flexible start and end times (approx. 6-8am start / 6-8pm end).

1+ years of paid experience in trach, G-tube care, and Trilogy /vent care, as well as medication administration and feedings.
Ability to start work at part-time (24hrs/wk) and build to additional hours over time (if needed, but not required).
About the Client:
This adult client has a loving, devoted family with both parents also serving as Caregivers, so on-site support is available.
The patient requires total care and support for all daily living needs, including bathing, dressing, feeding, assistance with routine maintenance exercises and passive range of motion, transfers, and other incidental services.

Starting Pay Rate:
LPNs: $28/hour
RNs: $35/hour
Position Summary
A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.
Reports to: RN Case Manager; Clinical Administrator/Director of Nursing
Qualifications
Graduate of an accredited school of professional nursing.
Current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio.
Minimum of one (1) year of experience in a home healthcare setting, acute care setting, or equivalent nursing experience is required. A passion for providing excellent in-home care is a must.
Experience in trach care, G-tube care, medication administration and tube feedings required.
Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide recertification if required.
Good interpersonal communication skills - written and verbal.
Current driver's license, good driving record, and reliable transportation.
Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
Excellent attendance record with past employers.
Essential Functions/Areas of Accountability:
Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
Promotes personal safety and a safe environment for clients and coworkers.
Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
Recognizes and responds appropriately to potentially unsafe situations.
Demonstrates safe practice in the use of equipment.
Makes recommendations on improvement of safety.
Performs job in compliance with agency policies and procedures and professional and community standards.
Accepts responsibility for professional nursing role.
Follows applicable state and federal laws and agency standards.
Attends all meetings as required.
Maintains confidentiality in all aspects of the job.

Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
Performs other related duties and responsibilities as deemed necessary.
