As an RN Home Health Clinical Manager, you will :

  • Be responsible for the oversight of all personnel and patient-centered care/services provided by the agency, including patient and personnel assignments, coordinator patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the patients’ individualized plan of care
  • Be responsible for the agency’s quality assurance and performance (QAPI) program, including patient outcomes and patient experience
  • Create a positive work environment conducive to the attainment of personal and business goals
  • Ensure overall compliance with all federal, state, and local government laws and regulations as well as policies and procedures of Interim HealthCare
  • This office has skilled services. The person in this role will be doing nurse visits in the field and working in-office.

To qualify as an RN Home Health Clinical Manager position with us :

  • A registered nurse with a license in good standing
  • Minimum of three (3) years of home health care experience preferred
  • Two (2) years of management experience preferred
  • Reliable transportation to/from care sites and/or work locations

NURSE CARE MANAGER - HEALTHCARE HOME
Springfield, Missouri

The Nurse Care Manager works in conjunction with the CPRC team to provide education and support to clients who have or are at-risk for complex medical conditions in addition to mental health and or Substance use disorders.

Job Duties:
Develop wellness and prevention initiatives.Facilitate health education groups by utilizing advanced knowledge of complex medical conditions.Participate in the initial treatment plan development for all of their Healthcare Home enrollees.Collaboratively develop a treatment plan of healthcare goals for individuals with co-occurring chronic diseases.Consult Community Support Specialists regarding patient identified health conditions.Manage the process of hospital and medical provider admissions and discharges for Healthcare Home patients.Train patients on complex health conditions regarding medical diagnosis, treatment, and medications.Implement, as required, DMH Net Health technology programs and initiatives such as CyberAccess and metabolic screening.Monitor and report performance measures and outcomes using sophisticatedd information systems developed for nurses.

Qualifications and skills:
Active RN or LPN license Experience in direct patient care preferred Knowledgeable in chronic disease processes in children and adults Excellent written and verbal communication skills.Ability to implement new innovative systems.Ability to make decisions based on critical thinking and nursing process

The Home Health and Home Hospice Nurse Manager is responsible for intake and coordination of care for hospice patients, staffing, problem recognition and resolution, staff development, budget, and customer service.
