Job Summary:

The Nurse Resident is responsible for providing and supervising direct and indirect total nursing care responsibilities to identified age-specific groups. Utilizes the nursing process (assessing, planning, implementing and evaluating) in achieving the goals of the nursing department. Nurse Residents and experienced nurses entering a training program will be under the direct supervision of a preceptor for all principal duties and responsibilities during the training program with eventual transition to independent practice after successful training program completion. Adheres to the AdventHealth Corporate Compliance Plan and to rules and regulations of all applicable local, state and federal agencies and accrediting bodies. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Responsibilities

The Disease-Specific Program Coordinator works with physician and administrative leadership to promote optimal patient care for a disease-specific patient population at a site-based level and to collaborate with peers across Legacy Health to promote integration of best practices throughout the organization. Serves as clinical expert resource for staff providing disease-specific patient care. Facilitates team coordination in the provision of evidence-based care. Develops and maintains disease-specific protocols and processes to support provision of care in conjunction with Service Line leadership (if applicable). Supports patient and community education. Prioritizes work and effectively manages time and projects with minimal supervision.

Qualifications

Education:

Academic degree in nursing (BSN or MSN/MN) required.

Experience:

Two years of experience in the provision and coordination of care for the disease-specific patient population required, 3 years preferred. Experience leading performance improvement efforts and disease-specific program management strongly preferred.

Skills:

Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, coaching, written and verbal communication. Proficiency in Microsoft Office Suite and clinical information systems.

The Clinical Program Coordinator is responsible for the oversight and coordination of dialysis care and treatment and promotes an environment to improve processes, optimize quality of care, increase patient access, enhance patient satisfaction, and reduce waste and expenses.

Lead and manage a systematic and coordinated approach to quality measurement and improvement initiatives impacting core clinical processes and critical patient care functions.

Work collaboratively with administrators, managers, the interdisciplinary team and physicians to identify and enhance strategic clinical issues, through the Quality Assessment and Performance Improvement (QAPI) process.
Support workgroups through planning, facilitation, task execution, and/or communication support. Implementation of new protocols and procedures. Assist workgroups with content and research support to progress the project. As initiatives progress, may engage with workgroups to identify external project support. Performance outcome deliverables include, but not limited to; quality, safety, satisfaction, service, operational efficiency, financial and process improvements. Represent the Dialysis Program throughout the UVA Medical Center, and in the local community. Participate in and facilitate physician peer-review and ongoing interdisciplinary and inter-professional collaboration within the Dialysis Program. Review of quality data on a monthly basis and actively addressing opportunities for improving dialysis care at UVA. Develop and review analytical work products for accuracy, completeness and help frame the analysis for leadership decision-making. Independently retrieve, interpret, and analyze data from a wide variety of sources. Accountable for the program's achievement of goals, mission, and objectives. Provides direct patient care as needed and provide support to other team members for complicated patient needs. Maintains a state of survey readiness which includes clinical audits, mock surveys and adherence to infection control practices. Coordinates interdisciplinary team rounds and participates as needed. Provides effective project consultation and facilitation to ensure projects meet goals and objectives as defined by leadership. Prepare and share reports with leadership to review status, important issues, and barriers that impact progress of all active and upcoming projects. May draw upon healthcare knowledge, trends, and experiences to execute tasks, inform analysis and evaluate issues for leadership discussion. May assist leadership to define project budgets and assist with the monitoring of progress against budgets. In addition to the above job responsibilities, other duties may be assigned.
Position Compensation Range: $67,849.60 - $108,555.20 Annual

MINIMUM REQUIREMENTS

Education: Bachelor of Science in Nursing from accredited nursing program.
Experience: Three years of progressive leadership experience which includes nephrology nursing background.
Licensure: Must be a registered nurse and currently licensed to practice in the State of Virginia

PHYSICAL DEMANDS

This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
