The Pediatric Home Care Educator is responsible for educating, orienting, and validating clinical staff who provide in-home nursing care to pediatric patients. This role ensures staff competency, regulatory compliance, and safe, family-centered care in accordance with Ohio regulations, hospital policies, and evidence-based pediatric practice.
Responsibilities:
1. Provide initial and ongoing education for pediatric home care nurses (RNs and LPNs).
2. Deliver pediatric-specific training including high-acuity skills such as tracheostomy and ventilator care, enteral nutrition, medication administration, seizure management, and infection control.
3. Design and deliver education using adult-learning principles (classroom, virtual, skills lab, and return demonstrations).
4. Complete initial, annual, and ongoing competency assessments per agency policy and Ohio requirements.
5. Validate clinical skills through observation, simulation, and return demonstration.
6. Provide remediation, coaching, and individualized education plans as needed.
7. Ensure education programs meet Ohio Department of Health (ODH), Ohio Board of Nursing (OBN), Medicaid, and accredited body requirements.
8. Support compliance with care plans, physician orders, and scope-of-practice standards.
9. Educate staff on clinical policies, procedures, and regulatory updates impacting pediatric home care.
10. Assist in development and revision of pediatric-specific clinical policies and skills checklists.
11. Track mandatory education and competency completion and report compliance to leadership.
12. Support preceptors and field staff with education, clinical guidance, and best practices.

Appropriately selects and performs patient education activities related to disease management/streamline processes and coordinates all course work 1. Assesses patient educational level, age, culture, language, length of stay LO s , motivation, and readiness to learn; religious practices; physical and emotional limits. Individualize teaching to ability of patient and family to comprehend and implement education. 2. Plans, implements and evaluates patient education activities in timely, efficient manner. Teaches patient/families about disease process, symptoms, medications, risk factors and management and home activity instructions. This is accomplished in the classroom, at the bedside, while ambulating and/or in one-on-one consulting sessions. 3. Provides appropriate educational materials for all patients in assigned areas and an ongoing educational plan for staff nurses to encourage educational documentation and staff participation in patient education.

Promotes continuity between in-patient and out-patient activities in facilitating rehabilitation referral process to out-patient programs 1. Initiates seamless referral system to all out-patient programs, including assessment, significant diagnostic test information and treatment, physician referral when appropriate e to decrease delays in program entrance. 2. Acts as member of multidisciplinary team for in-patient program. Gathers information from medical record, computer, observation, interviews, and staff rounds to promote seamless referral system. 3. Provides alternative method for communication and counseling for patients unwilling or unable to participate in Phase II Cardiac Rehab program to encourage symptom recognition and behavioral modification for risk factor modification. 4. Performs follow-up phone calls to discharged patients to assess compliance and needs for education and activity at home.
