    New York City Health and Hospitals Corporation Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City's efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. This is an incredible opportunity to be part of a passionate and motivated team providing care to some of the City's most marginalized, vulnerable people. *To help support continuity of operations and care, staff selected to work in the OTxHU may also be required to work in CHS locations within the jails. Additionally, while CHS seeks the most qualified individuals for these positions, preference will be given to equally qualified, internal candidates. Time: 08:00 AM - 05:00 PM Days: Mon,Tue,Wed,Thu,Fri Location: OTxHU- Bellevue/Rikers Island Under supervision of the Site Medical Director, the Physician will provide comprehensive, compassionate, and thoughtful care to patients with complex chronic disease in the New York City jail system. The Physician will be part of a core interdisciplinary team working in a unique environment delivering the care to patients with significant chronic illnesses. The Physician will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services. The interdisciplinary team will work under supervision of a Site Medical Director. Responsibilities include: Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated. Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable. Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits. Implement plans for patient care utilizing protocols approved by the medical leadership and/or treatment plans reflecting the current standard of care. Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate. Collaborate closely with CHS Physician Assistants, including providing clinical guidance, cosigning notes, and providing other supervision based on clinical circumstance and PA requirements. Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented. Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed. Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags). Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care. Ensure that all progress notes and orders are signed before the end of the shift. Respond to emergencies in a timely and professional manner. Notify the appropriate parties, including Urgicare, about 3-hour runs and EMS activation. Complete special housing rounds when assigned. Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards. Maintain clinical competency by participating in all CME and CHS training and in-service requirements. Maintain your schedule as directed with particular attention to punctuality and timely notification of absences. Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed. Complete tasks as delegated by a Site Medical Director or other supervising clinical team member. Maintain all required credentials. Maintain current licensure and CME requirements (Appropriate documentation must be on our files). Maintain professional attitude and appearance. Adhere to Occupational Health Services requirements. Minimum Qualifications 1. Graduation from an approved medical school. 2. Completion of approved residency or fellowship in the specialty or sub-specialty and Board eligible or certified or Subboard eligible or certified. 3. Five years experience in field of specialty or subspecialty acceptable to the Medical Board of the Hospital. 4. Licensed to practice medicine in the State of New York. The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools. Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes. Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. Administrative Responsibilities: Offer direct administrative and clinical supervision for the infectious disease service, encompassing ID clinicians and the Medspan team. Oversee staffing requirements for the infectious disease service within the facility. Direct the formulation and execution of workflows and protocols pertinent to the infectious disease service. Correctional Health Services The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Detailed Tasks: Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management. In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review. Clinical level judgement is needed in the daily supervision of radiological functions and patient care, and departmental projects. Responsibilities include: Review exam orders, consult with radiologist, schedule patients and arrange for transportation as indicated. Identify patient and document all information pertinent to final report in the RIS and on images (PACS). Prepare all patients (infants, children, adolescents, adults and geriatric) for radiological procedures requested by the clinician. Arrange for proper protection of patients, public and staff from infectious diseases/materials. Perform all general diagnostic imaging and routine examination in CT and Angiography. Assists in performing daily checks of radiographic imaging equipment, immobilization devices and other necessary accessories. Enters data and maintains documents and logs as required and mandated by the department QA guidelines and various regulatory agencies. Maintains adequate stocks of supplies and materials needed for use in the department. Checks equipment for defects prior for use and assures proper use of such equipment through instruction and review. Maintain basic care of all the imaging equipment. All malfunctions are to be reported to management staff whether mechanical, electrical or suspect and service request documentation completed. Maintain a clean and safe work environment for both patients and staff. Perform other duties as deemed necessary (by supervisory or administrative staff) in the event of emergencies or disasters. For Supervisory Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and two (2) years of Radiologic Technologist experience performing duties consistent with Assignment Levels I and II, one (1) year of which must have been in a supervisory capacity. 2. For Specialized Imaging Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and possession of a current advanced- level certification for the performance of these procedures. Ensures ongoing compliance with NYC Health + Hospitals and relevant health care setting policies, and other appropriate regulatory requirements. Essential Duties and Responsibilities: 1. Directs, coordinates, and maintains control over project assignments and related activities including in-house construction, renovation, and engineering activities. Follows standard methods and procedures for the implementation of maintenance systems and operations, construction contracts, and/or capital projects development. 2. Assists in the direction of trade staff through the development processes and procedures for the implementation of maintenance operations, construction contracts, and capital projects. Assists the Director in identifying staffing needs to support service delivery. 3. Prepares reports, develops and prepares engineering management audits, studies and surveys for the Director, and/or other senior management, detailing proposed new programs, statistical studies, and health care setting planning. 4. Assists in the collection of data and preparation of reports that outline the scope and estimated cost of projects proposed, implemented and developed. Organizes staff meetings, reviews capital and expense budget submissions and provides recommendations regarding proposed projects and programs. May represent the Director in budget meetings. 5. Plans and implements programs to establish performance standards to achieve increased productivity and accountability of trade staff. Reviews and evaluates existing programs, services, policies and procedures. 6. Provides departmental orientation, training and guidance to staff. Conducts in-service training programs, continually assessing staff training needs; suggests changes to Director to provide more efficient operations. 7. Coordinates efforts and cooperates with departmental leadership on matters of mutual concern and extends functional guidance and assistance. Meets with health care setting clients and staff to plan, prioritize, coordinate and/or communicate programs and policies, as needed. 8. May provide consultative services to senior leadership or liaise with Federal, State, and City agencies on matters pertaining to engineering, construction, design projects and/or other engineering support programs and functions. 9. In the Director's absence, may represent the System in certain legal matters dealing with State, City, and Federal agencies. 10. Performs other related duties, as directed. Minimum Qualifications Bachelor's Degree from an accredited college or university in Engineering, Architecture, Construction Management, Construction Engineering or a related discipline; and two (2) years of responsible-level experience in project, facilities or construction management, capital project planning, development, and design or other related experience in a hospital or health care setting; or High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization; and six (6) years of experience, as described in 1 above. Special Note: Valid license and current registration as a Professional Engineer or Architect issued by the New York State Education Department (NYSED) or a related Master's Degree can be substituted for one (1) year of experience. Position Overview Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to our patients in the New York City jail system. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services Responsibilities include: Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated. Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable. Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits. Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate. Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented. Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed. Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags). Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care. Ensure that all progress notes and orders are signed before the end of the shift. Respond to emergencies in a timely and professional manner. Notify the staff MD and Urgicareabout 3 hour runs and EMS activation. Complete special housing rounds when assigned. Be familiar with quality of care and population health indicators.
