    Join Aurora Mental Health & Recovery (AMHR) as a full-time Case Manager in a unique role serving the Older Adult Team, Elmira Counseling Center, and the Southeast Counseling Center (SECC). This position offers the opportunity to make a meaningful impact by providing comprehensive case management services to adults in outpatient settings, including individual, group and couples counseling. You'll work with a passionate team dedicated to supporting older adults and advancing care in the growing field of geriatric mental health, while also engaging with diverse clients in the Aurora community. Schedule. We offer flexible work arrangements, including either a 5-day, 8-hour workweek (Monday-Friday, 8am-5pm) or a 4-day, 10-hour workweek. Our hybrid model combines remote and in-office work, with an expectation of being on-site at least three days per week. This schedule may be adjusted based on client and community needs. The role will involve time at both the Hampden and Galena offices, in the community, and remote work as appropriate to meet client needs. Salary for this role. The starting wage is based on experience and company equity. Paid bi-weekly. $21.03 - $31.56 per hour Bilingual Language Differential Pay. *Rate depends on language requirements for the position and the candidate's fluency. Employees will receive scheduled earnings on a bi-weekly basis, equivalent to 5% of their regular base rate (hourly or salaried), with a maximum amount of $3,000.00 per calendar year. The annual maximum amount will be prorated based on full-time equivalency (FTE). Essential Functions: Some or all of the following essential functions will be relevant to the Case Manager role, depending on the specific clinical team where work is performed: Help clients in meeting basic needs for food, clothing, shelter, personal safety, and general health care, and assist them with applications for income, medical, housing, or other benefits which they may need and to which they are entitled. Ensure clients' access to needed services and community resources by arranging for transportation. Provide comprehensive psychosocial services designed to improve or maintain clients' abilities to function effectively. Help clients evaluate strengths and symptoms, and facilitate clients in setting their own goals and plan for appropriate services. Guide and instruct clients in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, use of transportation, and other community services. Help clients develop social skills, interests and leisure time activities, including opportunities for age-appropriate activities. Help clients find and make use of appropriate employment opportunities, vocational rehabilitation services, or supported work environments where appropriate. Offer support, assistance, consultation and education to families, friends, landlords, employers, community agencies and others who come into contact with clients, to maximize benefits and minimize problems associated with the presence of these persons in the community. Identify, work with, and make full utilization of potential natural support systems such as neighborhood networks, churches, and community organizations to encourage treatment program engagement. Reach out to eligible clients to inform them of and educate them about available services in the community. Provide transportation using agency vehicles as needed to help clients overcome barriers to accessing community resources. Qualifications Requirements: Bachelor's degree in psychology, social work, or other related human services degree. OR: Bachelor's degree in unrelated field with at least one year of behavioral health experience. Valid Colorado Driver's License with a driving record that meets organization's requirements. Demonstrated interest in behavioral health field. Ability to assess crisis situations and intervene appropriately. Knowledge of community resources. Basic understanding and ability to work with differing behavioral health presentations. Effective organizational skills. Effective written and verbal communication skills. In this role, you'll provide outpatient mental health care to adults with intellectual and developmental disabilities (IDD), including autism, who also experience co-occurring behavioral health conditions. Requirements: DD Case Managers must have: * A bachelor's degree in Social Work, Special Education, Psychology, Nursing, Human Services, or another field related to the education or treatment of persons with developmental disabilities or related conditions. If degree is in Social Work, Social Work licensure is required. * One year of experience in the education or treatment of persons with developmental disabilities or related conditions (QDDP qualified). * Completion of at least one course specifically focused on developmental disabilities (Fraser can provide this course free of charge). * Valid Minnesota Driver's License with acceptable record. * Ability to pass a DHS background study. * Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness, and reflection. CADI/BI Case Managers must have one of the following: * A bachelor's degree in Social Work, Psychology, Sociology, or a closely related field. If degree is in Social Work, Social Work licensure is required. * Or a bachelor's degree in any field and one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency. * And a commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness, and reflection.
