Reports outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of the evaluation. Participates in staff conferences and committees as requested by the agency. May supervise home health aides per LHC policy if allowed per state regulation. Provides supervision of the PTA as per LHC policy and state regulation. Schedule flexibility is needed in this role to ensure we provide care to the patient as needed. Requires the ability to participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients. Prepares a written discharge summary and/or a written summary report on patient's condition at least every 60 days. Provides in-service education as requested by agency leadership. Adheres to LHC Therapy Standards of Practice in provision of services to all patients. We are looking to coordinate and maintain holistic and synergistic relationships within each O2 Facility between Physical Therapy, Personal Training, Yoga, and Nutrition. We believe that by providing services in the actual gym, the Physical Therapist can easily communicate with the Personal Trainer, Yoga Instructor and other fitness professionals to ensure everyone is on the same page to help the client achieve their goals while continuing the activities they love. We provide 1-on-1 treatments focusing on outpatient musculoskeletal injuries and sports performance. In addition we provide Recovery Services (Infrared light therapy, Cupping, Dry Needling, Iontophoresis, Percussion therapy, IASTM, etc.) as well as Preventative Care and Maintenance.
