Position Summary: Responsible for the daily operation of the Health Information Management Department, which includes data entry, retrieval and scanning of health information into the electronic health records, tracking missing data. Job Responsibilities: Monitors (D)ocument, (L)ab, (R)eferral, & (T)elephone jellybeans assigned the HIM Department Handles incoming telephone calls Processes release of information pursuant to signed record release Processes incoming documents (ie. post office mail, fax server) Scans or attaches each document to patient's clinical record. Attaches to Lab/Xray order as needed. Distributes forms to be completed by provider to appropriate destination. Sets up alerts for legal documents. Scans “Old Records” into Chart Capture Scans outgoing forms completed by the providers into the EMR. Scans dental films into Open Dental Scans documents generated by office. Scans RHIO consents in eCW and RHIO. Checks each patient for documents/labs/xrays needed for next day appointment Follows up on the status of open referrals. Monitors “To Be Reviewed”/ “outstanding” labs Monitors the fax server to ensure that faxes are being received. Checks RHIO or lab company when e-results have not been received. Follows the state and federal laws and established OOH policies and procedures in regard to confidentiality issues in the release of health information and/or clinical records. Strict adherence to confidentiality of all information related to OOH patients, staff and organizational information. Disposes health information in designated confidential bins. Responsible for inactivating and reactivating records accordingly. Maintains good attendance and punctuality Any other reasonable requests from management. Job Responsibilities: * Maintains strict confidentiality sharing information with appropriate staff, following HIPAA guidelines and Center Policies and Procedures for release of information in the various internal and external agencies with whom you are working. * Provide interpretation for Spanish-speaking LLP (Limited Language Proficiency) patients during interactions with staff, appointments with OOCH providers and within the community for health-related issues. * Utilize the roles of medical interpretation, conduit, clarifier, cultural broker and advocate. * Provide written translation of OOCH forms, provider recommendations, or documents for patients as needed. * Conducts a needs assessment either through informal conversation, or through a standardized screening tool, such as the PRAPARE. * Identify any barriers patient may have such as low-level reading and/or illiteracy, and work to reduce these barriers. * Utilize local community resources to provide linkage for financial hardship and basic needs, allowing the patient to afford medication, insurance, OOCH sliding fee program, Rx Financial Assistance programs, food, clothing, and shelter, etc. * Assists patients with accessing health-related services, including making appointments outside of OOCH, ensuring the necessary arrangements for transportation, disabilities, illiteracy, interpretation, etc * Assists Case Manager to follow up on caseload and communicate with patients. * Registers new patients, schedules and confirms appointments at OOCH, as needed. * Assists patients with prescription renewals and refills, and paperwork for different health and human services programs. * Provide transportation to OOCH, outside providers and to community resources. * Works with patients and community agencies to increase awareness of health promotion and disease prevention programs. Develops effective working relationships to reduce barriers in the community for at risk patients. * Provides clinically approved educational materials in the appropriate language and at the appropriate literacy level, as directed by supervisor. * Work in tandem with field team staff to provide a link from outreach to in-office services. This position is responsible for scheduling future appointments both in person and via the telephone, registering new patients in the EHR, checks registration forms for completeness and accuracy, and scans registration into patient EHR. Job Responsibilities: * Accurately register new patients in the OOH electronic medical record (EMR) system, checking to see if the patient was previously registered and verify demographic and insurance information for accuracy. * Promptly and courteously, greet arriving patients and guests. * Promptly check patient in, including, but not limited to accurately updating insurance, yearly demographic information (registration form) and collecting any payments required from the patient. Promptly check patient out. * Process payments received via mail, phone, or in-person. * Schedules future appointments as needed, at both checkout window and over the phone. * Refer patients to Patient Accounts Office if financial plans are necessary. * Scans all completed patient paperwork into patient EMR in a timely manner. * Maintain phone etiquette; answer phones, transfer calls, take accurate messages and assign appropriately, document prescription refills and assign to correct provider. * Monitors appointment schedules * Respond to Patient/Physician inquiries * Reschedule patient appointments when providers call in sick or schedule time off. * Direct patients that need assistance with their health insurance to the embedded Facilitated Insurance Enroller. * Call appointment reminders for physicals and all new patient appointments and change appointment status to "confirm" when done. Call failed appointment reminder calls to attempt to confirm appointment. * The ability to communicate effectively with patients and staff from all departments. * Attend regular staff meetings, trainings, and other meetings as requested. * Maintain a safe, clean and orderly work area at the front desk and waiting room. * Schedule interpreters (internal or external) for patient appointments. * Balance cash receipts at the end of shift and prepare appropriate reports. * Make sure the building is unlocked in the morning, and locked at night with the alarm set by the last employee to leave the building. * Adheres to all organizational policies and procedures. * Any other reasonable requests from management. Assist in outside medical public relations functions. Adhere to Oak Orchard Health productivity standards and meet at least the minimum requirement. Lock all chart notes within seven (7) days of the patient visit. Essential Duties and Responsibilities: Provide comprehensive, evidenced based, behavioral health treatment services for patients. Provide biopsychosocial assessments, treatment planning, and clinical progress note documentation to support the therapeutic process. Develop an individualized treatment plan in collaboration with the client (and parent/guardian for pediatric patients) based on needs identified in the assessment. Maintain and complete clinical documentation for all clients in a timely manner. Collaborate with providers across an interdisciplinary team to provide integrated, well rounded care to patients. Engage in clinical supervision, team meetings, and clinical case review as needed and required. Provide crisis intervention services for clients in acute distress and collaborate with medical providers for warm handoffs. Establish strong relationships and communicate effectively with clients, family members, and staff. Comply with all administrative and clinical policies and procedures. Demonstrate, promote, and practice cultural competency towards clients and staff. Participate in after hours on call system rotation as needed and required. Participate in weekly group consultation with an experienced psychiatrist from URMC to support professional growth and development. The Medical Assistant (MA) is responsible for assisting the Clinician and Nurses in patient care. The Field Team RN will provide comprehensive nursing services to the Migrant and Seasonal Farm Worker patient population of the practice assigned to within Oak Orchard Health (OOH). The Field Team RN will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician, advanced practice practitioners, and other members of the multi-disciplinary team. The Field Team RN will work to their full scope of practice within the field. This position will require Tuesdays 3p-8p and Thursdays 3p-8p with options to pick up additional hours either supporting additional PES work or in the Oak Orchard primary care offices. Job Responsibilities: Assess and Screen in the field MSFWs needing triage: Determine necessity of in-office medical, dental, vision, behavioral health needs based on the presenting information/nursing assessment Answer patient questions and provide education within scope of licensure Administer vaccines/medications under standing orders of physician. Maintain up to date standing order(s) with assist of medical provider(s). Maintain and clean mobile unit between each patient as necessary Complete point of care testing as allowed. Provide outreach to farms and schedule visits in collaboration with the Field Team lead. Maintain stock of vaccines and supplies for the field team. Delegate tasks and work as a patient care team with Field Team LPN 1) Documentation Maintain accurate medical records. Complete documentation as events occurs. Maintain inventory and ordering of field team medications and medical supplies. Compile and submit monthly reports regarding services rendered. 2) Assist in population health efforts focusing on preventive care and chronic condition management: Utilize population health software and/or electronic medical record to create and provide reminders for health screenings-including A1C, PHQ-9, Smoking cessation, etc. Provide education in person regarding the importance of health screenings and health maintenance appointments 3) Participate as a member of the Care Team: Foster a working relationship with patients, providers, patient engagement services staff and practice staff Work effectively with others to coordinate patient and access care support services Provide input relating to changes that may enhance the practice effectiveness Participate in meetings and huddles as appropriate Provide feedback to providers regarding patient progress and barriers encountered 4) Other Clinical Support RN Functions and Responsibilities: Provide culturally competent care to all segments of our patient population Promote health literacy with all patient interactions Identify opportunities to improve processes and services Share issues that are obstacles to meeting patient needs with practice and leadership Maintain equipment supplies, verification of expiration dates, ordering. Job Responsibilities: Lead the Care Team in medical decision-making; provides supervision/collaboration to NPs and PAs. Perform HPI and physical examinations. Perform minor surgical procedures. Generate referral for patients requiring services outside the scope of direct primary care. Develops effective working relationships to reduce barriers in the community for at risk patients. * Provides clinically approved educational materials in the appropriate language and at the appropriate literacy level, as directed by supervisor. * Work in tandem with field team staff to provide a link from outreach to in-office services.
