POSITION DESCRIPTION:
The physician will serve as the primary care provider for a dedicated panel of geriatric patients.
Patients are located in long-term care facilities (ALFs, ILFs, SNFs) and private homes. Physicians are given a reasonable geographic area in which all patients are located (to ensure minimal daily drive time) and there is flexibility in regard to whether or not house call patients are included in the panel.
Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs. Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures as required and as settings permit
- educate patients and families on health management, disease prevention, etc.
- provide guidance on lifestyle modifications and self-care practices

The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only with very low volume. Full-time providers rotate the on-call responsibility, with a shift every 12 weeks or less.
