Physical Therapist Glendale AZ (10 miles NW of Phoenix) Urgently Hiring 4.5-day work week We are looking for a motivated Physical Therapist for our growing, multi-disciplinary pain management and orthopedic practice in Glendale AZ. This position is 100% outpatient care, providing exemplary patient care for established and new patients. Must be an excellent team player and work well with other providers in our team-based treatment approach. PT duties include conducting patient evaluations, treating a variety of musculoskeletal issues, ordering, and interpreting diagnostic tests, and communicating well with other providers to assist in the best care for patients. The ideal candidate will have experience in orthopedics, but recent graduates will be considered! A bonus would be someone trained in or willing to be trained in pelvic floor physical therapy.

Duties:
Conducting patient evaluations

Requirements:
Graduation from an accredited Physical therapy Program (DPT preferred)
National PT Board Certification
Experience working in out-patient, orthopedic care preferred but will consider recent grads
PT license in AZ
Pelvic floor PT experience or certification a huge plus (or those interested in training)
Schedule:
Monday - Friday (office hours end at 1pm on Fridays)
Full time- no weekends

POSITION SUMMARY
This position provides assessment, treatment, and education to cardiac and pulmonary patients and their families in an outpatient cardiac, pulmonary rehabilitation or inpatient setting. Writes exercise prescriptions to progress patients through phase two and three outpatient rehabilitation.

4. Under general supervision, documents patient treatment according to professional and regulatory standards.

6. Utilizes basic understanding of medical devices, equipment function and quality control measurements. Adheres to safety policies. MINIMUM QUALIFICATIONS
Requires a level of clinical knowledge as normally obtained through Bachelor's degree in Exercise Physiology or related field.
Minimum experience of a 300 + hour internship in cardiac rehabilitation or equivalent clinical/patient hours.
Requires knowledge in cardiac/pulmonary physiology, management and treatment of chronic disease, health promotion, ECG interpretation.
Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong interpersonal, communication and human relation skills.
Basic Life Support (BLS) certification is required.

Follows protocol with little deviation for patient assessments and exercise prescription. Promotes and may be a resource to team for standardization of exercise testing, department protocols and best practices. 5. Independently documents patient treatment according to professional and regulatory standards. Maintains strong competency in policies and procedures and all required documentation. May serve as a resource to other team members. Supports compliance to policies, procedures and regulatory standards. Assists with coordination of care from beginning to end to follow best practices.
6. Follows plan of care to accomplish functions within established procedures and scope of practice. Makes independent judgments based on specialized knowledge. Confers with supervisor on any unusual situations. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Physician offices, patient families, third party payers, and medical supply companies.
7. Utilizes solid understanding of medical devices, equipment function and quality control measurements. Adheres to safety policies. Troubleshoots equipment issues; notifies lead and manager if there is a need to communicate with vendor. Assists with follow up for repairs and maintenance as needed. Maintains a safe environment for patients, staff and visitors.
8. Complies with all ethical, legal and professional responsibilities. Maintains confidentiality of all patient information.
MINIMUM QUALIFICATIONS
Possesses a level of clinical knowledge as normally obtained through Bachelor's degree in exercise/physiology or related field.
Minimum of 1 year experience in cardiac and/or pulmonary rehabilitation and/or stress testing.
Minimum experience of a 600 + hour internship in cardiac rehabilitation or equivalent clinical/patient care hours.
Requires knowledge in cardiac/pulmonary physiology, management and treatment of chronic disease, health promotion, ECG and 12-lead interpretation.
Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong interpersonal communications and human relation skills.
Basic Life Support (BLS) certification and Advanced Cardiac Life Support (ACLS) certification required.
PREFERRED QUALIFICATIONS
Master's degree in Exercise Physiology or related field
American College of Sports Medicine (ACSM) clinical exercise physiologist (CEP) certification
Up to 1 year experience in stress testing
Additional related education and/or experience preferred.
