About this Position:
As an OccupationalTherapist in our outpatient clinics, you will have the time and resources to provide one-on-one carewith 45-minute treatment sessions, ensuring high-quality interactions and better patient outcomes. With a maximum of 10 patients per day, you'll have the opportunity to focus on delivering personalized care without the burden of overwhelming caseloads. We also offer flexible scheduling options, including weekends and holidays off, to help you maintain a healthy work-life balance.
In this role you will be responsible for examining, evaluating and testing individuals with mechanical, physiological and developmental impairments, functional limitations, and disabilities or other health and movement-related conditions in order to determine a diagnosis, prognosis and plan of treatment intervention, and assess the ongoing effects of intervention. The OT is also responsible for alleviating impairments, functional limitations and disabilities by designing, implementing and modifying treatment interventions. Additional responsibilities include reducing the risk of injury, impairment, functional limitation and disability, including the promotion and maintenance of fitness, health and wellness in populations of all ages. The OT will be engaged in administration, consultation, education and research. The OT will be expected to conduct themselves in an effective professional behavior manner. The attributes, characteristics or behaviors required for success in this job include: critical thinking, communication, problem solving, interpersonal skills, responsibility, professionalism, use of constructive feedback, effective use of time and resources, stress management, and commitment to learning.

* Utilize thermal and electrical physical agent modalities not limited to providing hot/cold packs, fluidotherapy, paraffin/contrast baths, ultrasound, phonophoresis, TENS, NMES, HVPGS, iontophoresis and biofeedback. Fabricate custom splints, select/order adaptive devices/equipment and train client in use.
* Communicate effectively with fellow staff regarding client's condition, which may include their physician, rehab nurse, case manager, employer and family members to ensure consistent quality occupational therapy services.
* Provide a written status report to referring physician each time client is to see their physician and when is a workman's comp case to other appropriate parties.
* Assist on occasion with Industrial Service Department clients.
Minimum qualifications for the ideal future caregiver include:
* Bachelor's, Master's and/or Doctoral Degree from an accredited school of Occupational Therapy
* Occupational Therapy (OT) license in the State of Florida

Job Summary and Qualifications
The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. What you will do in this role:
You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
You will provide purposeful and skilled therapeutic care using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
You will instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
You will prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

Associate's degree from an accredited Physical Therapy Assistant program

Your Role as a Speech Therapist:
Provide speech therapy services to patients in their homes on an intermittent basis.
Conduct thorough assessments to evaluate speech, language, and swallowing conditions.
Develop personalized, goal-oriented treatment plans in collaboration with patients and caregivers.
Deliver therapy based on physician orders and care plans under the supervision of the Director of Clinical Services or Clinical Manager.
Educate patients and caregivers on strategies to improve communication and swallowing function.
Qualifications:
Active and unencumbered Speech-Language Pathologist license in the state of practice (required).
Medicare OASIS documentation proficiency (required).
Valid driver's license, reliable transportation, and auto insurance (required).
Two years of experience in speech therapy (preferred).

As a Home Health Speech Language Pathologist, you will:
Evaluate, direct and provide speech/language pathology service to patients in the home or facility
Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
Utilize professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician.
Provide instruction and training to patients in use of alternative communication systems when appropriate.
Provide counsel and instruction to patients, families and healthcare staff.
Maintain and submit documentation as required by the Company and/or facility. Prepare and submit timely written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports as required by Company policy.
Participate in care coordination activities and discharge planning.
Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is clearly in the best interests of the patient.
Attend, participate in and/or conduct internal staff development programs, obtain continuing education as required by Company policy, regulation.

Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
Minimum of six months experience as a speech therapist / speech language pathologist
Home Health experience a plus
Current and unrestricted license
Current CPR certification
Good organizational and communication skills
A valid driver's license, auto insurance, and reliable transportation are required.
Pay Range
• $49.00 - $69.00 - pay per visit/unit
• $77,200 - $106,200 per year base pay
