Provides physical therapy consultation to home families/caregivers when indicated.
Communication:

Supervises Physical Therapy Assistants according to organization policy and state regulations.
Instructs, supervises and evaluates home health aide care provided every two (2) weeks when therapy is the only skilled service
POSITION QUALIFICATIONS
1. Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
2. Licensed to practice as physical therapist within the State, if applicable.
3. Two years of appropriate experience as a physical therapist. Community/home health experienced preferred.
SKILLS REQUIRED
1. Demonstrates good verbal and written communication and organization skills.

3. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is

Assessing Patients: Conduct evaluations of patients' physical, emotional, and cognitive abilities, as well as their environment, to determine the necessary therapeutic interventions.
Developing Treatment Plans: Create individualized therapy plans based on the assessment, working with patients to set goals and choose appropriate interventions.
