Responsibilities
Evaluate and conduct medically prescribed physical therapy treatment programs.
Perform initial and on-going assessments of patient's condition.
Establish, revise and evaluate a plan of care which is appropriate to problems identified and involves the patient/family.
Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
Oversee activities of physical therapy assistants, students and other support personnel.
Requirements
Licensure / Certification / Registration:
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Licensed Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Executive Council of Physical Therapy and Occupational Therapy Examiners obtained within 6 Months (180 days) of hire date or job transfer date required.
Education:
Bachelor's degree required.
Master's degree preferred.
Additional Preferences
Acute care experience preferred (clinical or student experience).
Emergency department, critical care units, and/or vestibular experience preferred.

The Clinical Exercise Physiologist 1 coordinates and plans preventative and rehabilitative health care for patients. This position provides care by observing and assessing the patient's condition. It involves performing interventions and testing, evaluating care results, and educating patients and families about the condition and treatment.
Essential Functions of the Role
* Provides patient care by observing and assessing the patient's condition. Performs patient care interventions and evaluates the results of care. Exercises and administers medications. Educates patients and their families about the condition, care, and treatment.
* Assess the patient's condition by obtaining their medical history. Use written reports, interviews with the patient or family, and observation.
* Develops exercise prescriptions based on the patient's condition and medical plan. Performs exercise tolerance testing on various patients, following guidelines. Implements care by teaching the patient prescribed exercises. Reassesses the patient's response and modifies the exercise plan as needed. Works closely with team members to coordinate care.
* Documents important assessment information, treatment activities, and patient outcomes in the patient record. Informs team members and family spokesperson of important changes in the patient's condition.

Job Responsibilities:
Perform ongoing clinical assessments to determine patient needs.
Evaluate patients' occupational performance in self-care, work, leisure, and rest.
Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.
Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
Document clinical notes, phone orders, and other records promptly and accurately.
Provide education and counseling to patients and caregivers.
Supervise and guide occupational therapy assistants and home health aides as necessary.
Apply infection control and standard precautions during patient care activities.
Communicate effectively with physicians and healthcare practitioners involved in the patient's care.
Meet continuing education requirements and demonstrate professional growth and competency.
Promote the Agency's philosophy and policies to ensure high-quality care.
Environmental and Working Conditions:
Work in patients' homes under various conditions, with possible exposure to blood, bodily fluids, and infectious diseases.
Ability to work a flexible schedule and travel locally, with occasional exposure to unpleasant weather.
Requirements
Education: Bachelor of Science in Occupational Therapy.
Licensure: Active state license as an occupational therapist or meets the requirements listed in 42 CFR ยง484.115(f). Military spouses with an equivalent out-of-state license are welcome.
Experience: A minimum of one year of home health experience preferred.
Skills: Proficient therapy skills according to accepted standards of practice, good interpersonal skills, CPR certification, knowledge of durable medical equipment.
Software Knowledge: Familiarity with HomeCare HomeBase software is a plus.
Transportation: Reliable transportation, valid auto liability insurance, and current driver's license.

We are looking for a committed and patient -focused Occupational Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions.
You will offer effective and client -centered services that will enable people to confidently and independently carry out everyday tasks or activities. The successful candidate will have a holistic perspective and will be able to adapt the environment to fit the person.
Address client's aspects of performance to support engagement in occupations that affect health, well -being and life quality
Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions
Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components
Assess client's home and other environments and advice on alterations consistent with their needs and capabilities
Recommend adaptive equipment and train in its use
Guide and educate family members and caregivers
Observe, note, and report on progress of long and short term treatment goals
Collect data and document processes followed
Regularly follow -up client
Requirements
Proven working experience as Occupational Therapist
Excellent knowledge of principles and practices of occupational therapy
Hands -on experience with computer programs and creative media
Familiarity with ADLs evaluation tools ( e.g. Katz ADL scale, Lawton IADL scale)
Ability to manage patients with different types of personalities
Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession
