Role Highlights
Anticipated Start Date: 3/1/26
Compensation: $120 per Evaluation
Employment Type: Contracted, minimum of 10 hours of weekly availability
Location: In-Home Visits
Key Responsibilities
Conduct in-home assessments and functional evaluations
Deliver personalized, evidence-based recommendations for home modifications
Educate clients and caregivers on fall prevention and safe mobility techniques
Engage clients and families in collaborative care planning
Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives
Maintain accurate and timely documentation through our electronic system
Qualifications
Degree in Occupational Therapy from an accredited program
Active, unencumbered OT license
2+ years of clinical experience
Experience in home health, aging in place, or disability services preferred
Strong interpersonal and clinical reasoning skills
Reliable transportation and valid driver's license
Proficiency with technology and digital documentation
Ability to work independently while collaborating with a remote team
More About Compensation
Contractors are paid on a monthly basis, within 2 weeks of the previous month's end.
We pay per completed evaluation - this includes:
Travel to/from client's home
The evaluation itself
Documentation

Job Description
The primary role of the Speech Therapist is to provide services to clients in the Pediatric (95%) and Adult (5%) programs. This role will be responsible for the evaluation of the client and the implementation of the treatment plan. The Speech Therapist will also be responsible for appropriate communication (verbal & written) with service coordinators, intake/billing specialists, family members, insurance companies, and physicians. Speech Therapists in this setting require significant pediatric dysphagia experience or continuing education.
DUTIES AND RESPONSIBLITIES:
Complete a thorough evaluation and identify needs in the areas of pre-speech, cognition, feeding and speech and language development, and augmentative communication/assistive technology as appropriate.
Educate and demonstrate (verbally or written) therapy techniques to clients that can be incorporated into a home program; assist families in home program facilitation.
With client input, develop individualized goals for each patient, incorporating functional abilities with the objective to increase daily living skills.
Develop and implement treatment program to meet the individual needs of the client. Treatment provided is primarily center-based for clients. Teletherapy and home therapy occur on occasion.
Attend staff meetings, in-service training, and continuing education courses as appropriate or required.
Maintain communication with client's primary physician through progress reports or other communication measures.
